Objective:To investigate the early outcomes of patients with multilevel ossification of the posterior longitudinal ligament receiving anterior cervical corpectomy and fusion,posterior cervical double-door laminoplasty and lamina staple fusion.Methods:The clinical data of 56 patients with multilevel OPLL receiving surgery in Sandong TCM Hospital from January 2011 to November 2014 were retrospectively studied,and all patients were followed up for at least 3 months.Twenty-six cases received posterior cervical double-door laminoplasty and lamina staple fusion(group A),30 cases received anterior cervical corpectomy and fusion(group B).The operation time,intraoperative blood loss,postoperative complications,JOA scores,cervical curvature index were comparatively studied.Results:After the surgery,the JOA scores were all significantly improved.Group A and group B were not comparable regarding the operation time and intraoperative blood loss.Group A had less postoperative complications than Group B after the surgery.Group B had an improvement in CCI after the surgery,and the improvement in CCI are proportionate to the CCI before the surgery.Group A has no change on CCI after the surgery.Conclusion:The two approaches are effective for the treatment of multilevel OPLL.Posterior surgery was more safe than Anterior surgery. Anterior surgery has advantages in cervical curvature index improvement,while posterior surgery has no changes in cervical curvature index improvement. |
