Objective:With a variety of modern research techniques and tools,this experiment is to study the effect of acupoint catgut embedding therapy and acupoint injection therapy on inflamma tory markers and insulin resistance in diabetic rats,and to observe the morphological changes of islets,meanwhile try to find the mechanism of two kinds of acupoint therapy for anti-diabetic inflammatory and improving insulin resistance,and comparing the effect between the two kinds of acupoint therapy,to explore the new ways for clinical application of acupuncture in the treatment of diabetes and to provide experimental evidence for further study. Methods:1.Grouping:Within fifty Wister male rats,selecting ten in random as normal control group.Into the experiment after being fed for one week.The other fourty rats as the DM models by injecting streptozotocin into their abdominal cavity.Giving normal diet to normal control group.Injecting streptozotocin into the other groups after 4 weeks of high fat diet.Collecting venous blood from the tails to examine the empty stomach blood sugar after 72 h,and the measurements were repeated 2 times. Fasting blood glucose which is more than 11.1mmol/L values as successful DM rat models.The 33 successful DM rat models are divided into three groups randomly:model control group(9);acupoint catgut embedding group(10);acupoint injection group(11).2.Treatment:normal control group and model control group were not treated.Refer to“Rat acupoint atlas”developed by Hua XingBang,acupoint catgut embedding group and acupoint injection group select“zusanli”,“weiwanxiashu”and“sanyinjiao”.The method of operation of acupoint catgut embedding group:after disinfecting the points with 75% medical alcohol,The medical surgical catgut cut to 0.3-0.5cm length,from 7catgut embedding needle is inserted into the needle with the right hand,pinch the thumb and middle finger food,skin,needle and a 15 degree angle to the rapid downward into the needle.Then insert surgical catgut into the needle and needled into the corresponding acupoints.One times a week,continuous treatment and observed for 4 weeks.The method of operation of acupoint injection group:The needle rapidly pricked into the corresponding acupoints after disinfecting the points with 75% medical alcohol,then lifting and thrusting of needle.By extracting,no blood returns,then injecting drug into the corresponding acupoints.The injected dose of each acupoint is 1ml,once every other day,4 weeks as a treatment course.3.Testing:Before and after treatment,collecting venous blood from the tails to examine the empty stomach blood sugar.Taking blood from abdominal aortic after the completion of treatment,the serum insulin is tested with radioimmunoassay(RIA),and Elisa is used to test TNF-α、Leptin and Adiponectin.To investigate the effect and mechanism of acupoint therapy in the treatment of type 2 diabetes.Providing scientific experimental basis for clinical application of this therapy in the treatment of type 2 diabetes,and to observe the changes of pathological morphology of pancreatic tissue by HE staining of pancreatic tissue of rats. Results:1.Therapeutic effect on diabetic rats:The amount of food,drinking and body weight and other symptoms of acupoint catgut embedding group and acupoint injection group improved significantly compared with the model control group.After treatment,fasting blood glucose levels in the two treatment groups significantly decreased(P<0.01),and serum insulin levels were significantly lower than the model group(P<0.01).Comparison of model group increased insulin sensitivity index(P<0.01),and insulin resistance index decreased compared with the model group(P<0.01).And the above indicated acupoint injection group was better than that of hypoglycemic effect of catgut embedding group,the difference had statistical significance.2.The effects of TNF-α、Leptin and Adiponectin in diabetes rats:Compared with the model group,the two treatment groups reduced the level of TNF- α and Leptin,and increased the level of Adiponectin(P<0.01).Prompting its therapeutic mechanism might be they affect the level of serum inflammatory factor.And the therapeutic effect of acupoint injection group was slightly better than the acupoint catgut embedding group(P<0.05).3.Pancreatic islet morphology observation:in normal control group,it can be seen that the islet boundary is clear,the island’s cytoplasm is full,and with large and orderly islet cells.The islet boundary in model group is not clear,and the island’s cells decreased,arranged in disorder with vacuolar degeneration.The acupoint catgut embedding group and acupoint injection group had different degrees of improvement.Compared with the model group,the boundary is more clear,the number of cells is more with a small amount of vacuolar degeneration. Conclusion:1.The two kinds of acupoint therapy have a good curative effect on diabetic rats.They can improve polydipsia,polyuria and weight loss and other symptoms,and can reduce the level of blood glucose and serum insulin.And then improve the sensitivity of tissues to insulin and reduce insulin resistans.2.The two kinds of acupoint therapy can improve the body’s inflammatory response in diabetic rats by affecting inflammatory cytokines,reduce insulin resistance.3.The two kinds of acupoint therapy can increase the number of islet cells,inhibition of islet cell apoptosis,to improve pancreatic β-cell function in type 2 diabetic rats.4.There was statistically significant difference in efficacy between acupoint catgut embedding therapy and acupoint injection therapy.In lowering blood glucose,improving insulin Resistance and islet β-cell function and other aspects,the effect of acupoint injection therapy is more significant.5.These results for acupoint therapy used in the treatment of diabetes and its complications to provide a scientific basis for experiments.It provides in addition to oral medication and insulin therapy more options for type 2 diabetes.Because of its simple operation,convenient use,low cost,obvious effect,no side effects and adverse reactions,it is worth promoting in clinical practice.
