Inflammation plays an important role in the formation, development and rupture process of coronary atherosclerotic plaque. At present, a large number of markers of inflammation associated with the stability of plaque had be found. Among them, Interleukin-6 receptor(IL-6R) can combine interleukin-6(IL-6) form complexes play a role through the gpl30 signal transduction receptor which in is rich in myocardium cell, is an important inflammation marker of atherosclerosis. Adisintegrin and metalloprotease with thrombospongding type 1 motifs(ADAMTS-1) mainly through up-regulating proliferation, migration, apoptosis of vascula smooth muscle cell, to activate different cell surface molecules, participate in the degradation and recombination processes of extracellular matrix, play an important role in the initial、development and rupture process of coronary artery atherosclerotic plaque. High sensitivity C reactive protein(hs-CRP)is an acute reaction protein formed when the acute inflammation IL-6,tumor necrosis factor(TNF) and other cytokines stimulates the liver, is an independent risk factor of coronary heart disease. This research through the observation of the relationship between the peripheral levels of IL-6R,ADAMTS-1, hs-CRP and Gensini score, Ambrose classification of patients,to investigate the relevance of IL-6R, ADAMTS-1 and hs-CRP to unstable plaques. Also investigate the effect of PCI on markers of inflammation by observing the levels of IL-6R, ADAMTS-1 preoperative and postoperative of PCI in patients of acute coronary syndrome.328 patients underwent coronary angiography were admitted, 272 patients were diagnosed as coronary heart disease. Including 105 cases of AMI, 126 cases of UA and 41 SA patients, selected a control group of 56 cases with no coronary artery stenosis. ACS includes AMI and UA. AMI group including PCI AMI 39 cases; UA group including PCI UA 42 cases.All samples were collected blood immediately hospitalized and PCI ACS patients were collected blood 12 hours after PCI. Detected the levels of IL-6R, ADAMTS-1 by Elisa and detected hs-CRP, TC, TG, LDL-C in Hitachi 7170 A automatic biochemical analyzer. Then evaluate the Gensini score by coronary angiography, and appraise the character of the plaque by Ambrose classification. The results show that: the serum IL-6R,ADAMTS-1 levels of group AMI and UA were significantly higher than that of group SA and normal control group(P<0.05); hs-CRP levels of group AMI were significantly higher than that of group UA, SA and normal control group(P<0.05); These index of complex lesion evaluated by Ambrose classification were significantly higher than that of simple lesion group and normal control group(P<0.05); ADAMTS-1 level of simple lesion group was significantly higher than normal control group(P<0.05); the level of IL-6R of ADAMTS-1 in group of coronary heart disease had Significant positive correlation by Pearson correlation test(r=0.70,P<0.05); The serum IL-6R, ADAMTS-1 and hs-CRP levels had no relationship with Gensini score. The IL-6R and ADAMTS-1 levels had no statistical difference in multivessel disease group, the double-vessel disease and single-vessel disease group(P>0.05). The serum IL-6R,ADAMTS-1 in group PCI AMI and PCI UA after 12 h of PCI increased compared with preoperative, the difference was statistically significant(P<0.05).The IL-6R, ADAMTS-1 and hs-CRP levels have significant correlation with the unstable plaque, but don’t relate to the severity of coronary artery stenosis; IL-6R and ADAMTS-1 levels increased after PCI in patients of ACS, IL-6R, ADAMTS-1 can predict plaque instability.
