Objective:To investigate the effect of local infiltration anesthesia on the s afety, feasibility and analgesic analgesia in pediatric surgery postoperative effect. And non steroidal anti-inflammatory analgesic acetaminophen in preemptive an algesia compared to two kinds of drugs, and each 6h added a paracetamol sup pository, an additional two times, in order to seek a kind of side effect more s afe, painless, and reasonable, at least, the best analgesia method applicable to c hildren after surgery.Methods:the implementation of pediatric surgery under general anesthesia (hydrocele of tunica virginals testis fixation,orcheopexy,cryptorchidism,inguinal h ernia, appendicitis, pyeloureteroplasty) were 90 cases, ASA grade Ⅰ-Ⅱ, Patients were randomly divided into three groups,30 cases in each group. The design scheme is as follows:I group at the end of the operation incision infiltration with 0.33% ropivacaine 3-5ml on both sides and in the postoperative 6h and 1 2h were added a paracetamol suppository 20mg/kg; group Ⅱ in operation befor e the ward nurses were given by the paracetamol suppository 40mg/kg,6h and 12h after operation each additional one acetaminophen suppository 20mg/kg. P reoperative Ⅲ group with group Ⅱ in the ward for the end of paracetamol sup pository combined operation with 40mg/kg incision infiltration with 0.33% ropi vacaine on both sides of the local 3-5ml,6h and 12h respectively after another paracetamol suppository 20mg/kg. Record the blood dynamics index during the operation and anesthesia recovery time. Observation of 3 groups of patients w ith 0.5h (T0),1H (T1),2h (T2),4H (T3),6h(T4),12h (T5),24h (T6),48h (T 7) FLACC and FPS score. Postoperative follow-up, record adverse reactions (ir ritability, crying, nauseaand vomiting, lethargy, pruritus), and incision healing.Results:(1) after T0, T6, T7 time point of the FLACC and FPS scores had no significant difference. (2) were T1, T2 and T3 at different time points of postoperative analgesia, group Ⅱ was higher than that in Ⅲ group, there was difference; group Ⅱ than group Ⅰ, but the difference was not statistically significant; compared with group Ⅰ and group Ⅲ, no significant difference. (3) two kinds of pain score T4, T5 time points after operation, in group Ⅲ was lower than that of group Ⅰ and group Ⅱ, there was difference between group Ⅰ and group Ⅱ; comparison, no statistically significant difference. (4) in group 2 cases (6.67%), restless crying in group Ⅱ in 3 cases (10%),1 cases in Ⅲ group (3.33%) (5) In group 1 cases of nausea (3.33%) in group Ⅱ and group Ⅲ 3 cases (10%), there was no vomiting. No case of lethargy, respiratory depression or skin itching symptoms in the three groups. (6) In three groups after operation incision healed, no case of infection of incision, group Ⅱ only 1 case had incision hematoma, but did not affect the release and discharge.Conclusions:1. The concentration of 0.33%,Ropivacaine local infiltration for pediatric comm on finish operation incision pain after surgery and preoperative application of p aracetamol suppository analgesia in advance also has good analgesic effect.2. Preoperative paracetamol suppository analgesia joint surgery biro pp in advance because local infiltration of postoperative analgesia.Effect is more apparent.
