Objective:The Surgical operation is one of the most important and effective means in the treatment of lung cancer at present.The Acute atelectasis is a common complication after resection of lung cancer, which seriously affects the quality of life of patients with lung cancer during peri operation period and survival rate. Through fiberoptic bronchoscope in the treatment of acute atelectasis after lung cancer surgery, were retrospectively analyzed the mechanism of the occurrence of pulmonary atelectasis, pulmonary atelectasis in the high-risk factors and bronchoscopy in lung cancer postoperative atelectasis clinical effect.Methods:The Third Affiliated Hospital of Kunming Medical University, Department of thoracic surgery from 2013 January - 2015 February lung cancer during postoperative acute atelectasis in 65 cases,for bronchoscopy in pulmonary atelectasis, were observed before treatment and after treatment for 1 hours,24 hours after treatment, treatment after 48 hours of oxygen saturation, heart rate, respiratory rate, oxygen partial pressure, carbon dioxide partial pressure changes.Statistical analysis all data were statistically analyzed using SPSS20.0 software package, comparison between groups with t test, measurement data were compared with the mean+standard deviation (x ± S) to say, in P<0.05 the difference was statistically significant.Results:In 65 patients with lung cancer after operation of acute atelectasis were underwent bronchoscopy after treatment retrospective analysis showed: pulmonary atelectasis occurs, the upper lobe of the right lung in 5 cases, accounting for 7.69%, middle lobe of right lung in 16 cases, accounting for 24.62%, inferior lobe of right lung in 12 cases, accounting for 18.46%, right lung lower lobe in 11 cases, accounting for 16.92%, right lung in 3 cases, accounting for 4.62%; the upper lobe of the left lung in 6 cases, accounting for 9.23%, left lower lobe in 10 cases, accounting for 15.38%,2 cases of left lung,accounting for 3.08%; Pulmonary atelectasis occurred in the age distribution,36-46 years old were 9 cases, accounting for 13.85%,47-57 years old were 12 cases, accounting for 18.46%,58~68 years old were 23 cases, accounting for 35.38%,69-79 years old were 21 cases, accounting for 30.31%; the incidence of atelectasis in patients with smoking, smoking in 36 cases, accounting for 55.38%, no smoking patients accounted for 29 cases, accounting for 44.62%; The operation mode, conventional thoracotomy operation in 50 cases, accounting for 76.92%, underwent thoracoscopic operation in 15 cases, accounting for 23.08%, after treatment by fiberbronchoscope examination results, combined with image, evaluation of lung recruitment excellence were 43 cases, accounting for 66.15%, effective in 19 cases, accounting for 29.23%, invalid in 3 cases, accounting for 4.62% of the total, efficiency in 95.38%.Conclusions:The postoperative acute lung atelectasis of the risk factors,include elderly patients, smoking status, operation mode,operation situation, anesthesiologists, postoperative pain.For acute atelectasis underwent bronchoscopy in the treatment of lung cancer after surgery, can quickly clear the nature of airway obstruction, obstruction, accurate, safety relief of airway obstruction and treatment. With the advantages of small trauma, quick effect, high safety, high rate of lung recruitment, repeated operation sex is strong, easy to clinic, can shorten the hospitalization time, reduce the incidence and mortality of lung cancer in peri operation period of pulmonary atelectasis.
