Risk Factors Analysis On 22 Cases Of Dermatomyositis Associated With Malignancy In Dalian City

Objective: Dermatomyositis is erythema, edema in skin lesions, accompanied by muscle weakness and muscle inflammation, degenerative diseases, mainly involving the skin and blood vessels, often accompanied by multiple organ damage, such as joint, myocardial and so on. All age groups may be involved. Main cutaneous manifestations are pruritic, symmetric, confluent, macular violaceous erythema variably affecting the skin overlying,most prominent the upper eyelids,and the extensor aspect of the face and upper chest, the central aspect of the face, periorbital areas, the forehead,neck,eras and the scalp. Gottron papules and sign are cutaneous manifestations also.There is a correlation between Dermatomyositis and malignant tumor reported by Sterez in 1916. A lot of case reports of dermatomyositis complicating malignancy emerge in recent years. To summarize and analyze the high risk factors related to dermatomyositis complicating malignancy. People usually study dermatomyositis and polymyositis together, but the results can not be fully representative of the characteristics of dermatomyositis, in particular for cutaneous manifestation. In elderly people with dermatomyositis is easily in malignant tumors, Especially the people who are over 40 years old. A variety of malignant tumors can occur, such as nasopharyngeal cancer, lung cancer, bladder cancer, breast cancer, colorectal cancer, ovarian cancer, lymphoma, etc. In this study, we analyze and summarize the high risk factors with dermatomyositis complicating malignancy, in order to improve the understanding of malignant tumors with the disease in the clinical work, to avoid missed diagnosis.Methods: Clinical and laboratory data of 22 patients in Dalian area(1999-2014)with Dermatomyositis associated with malignancy were collected and analyzed.We analyzed the malignant tumor types and the relative time relations between the cancer and dermatomyositis onset,39 cases of dermatomyositis patients(1999-2014) in Dalian without malignant tumor as control group; and analyzed clinical manifestation and laboratory examination. Statistics software SPSS 13.0, X2 test and Fisher’s exact test were used for the risk factors analysis on dermatomyositis associated with malignancy in Dalian city.Results: There are many types of dermatomyositis associated with malignant tumor.Such as lung cancer, colon cancer, bladder cancer,liver cancer,ovarian cancer,malignant teratoma,breast cancer, gastric cancer, nasopharyngeal carcinoma and diffuse large B cell lymphoma. Lung cancer is present in a highst proportion. Malignant tumor and dermatomyositis occur at the same time, which is accounted for 45.45%.Statistical analyses revealed that the high risk factors of dermatomyositis associated with malignancy are male,more than 40 years old,malignant erythema, poikiloderma,dysphagia, LDH increased, carbohydrate antigen CA199 and CA125 increaed.Conclusions:1. These results provide evidence that dermatomyositis is often associated with malignant tumor.Lung cancer is present in a highst proportion in Dalian according this study.2. The main risk factors of dermatomyositis associated with malignancy may be male, more than 40 years old, malignant erythema, poikiloderma, dysphagia,carbohydrate antigen CA199 and CA125 increased.
