Analyze The Clinical Data Of Postoperative Hematoma For Sphenoid Ridge Meningioma

Objective:analyze the diagnosis, surgery treatment and postoperative hemato ma with sphenoid ridge meningiomas (SRMs) to summarize the reasons of posto perative hematoma. to further improve the rate of safe tumor resection.Method:To statistic the clinical data of 45 patients with sphenoid ridge meni ngioma underwent operational treatment in THE FIRST AFFILIATED HOSPITAL OF DALIAN MEDICAL UNIVERSITY during 2008 -2014. At the suggestion of professor Watts, in accord with inner SRMs has 29 cases and outer SRMs ha s 16 cases. According to the tumor diameter size in imaging, the smair"SRMs h as 13(d≦ 4.5cm), the large (4.5<d≦ 7cm) SRMs has 28 and the giant SRMs h as 4 (d>7cm). Taken CT, MRI for patients and some patients CTA. All 45 patie nts were treated by microsurgery , by pterion approach has 38 cases, and impro ved pterion approach has 7 cases. Excellent hemostasis was obtained. To sum up the extent of tumor resection, postoperative hematoma complications.Result:According to the situation in microsurgery and image processing after operation.The extent of the tumor resection was evaluated according to Simpson classification method, Simpson grade Ⅰ was resected in 21 cases,Simpson grade Ⅱ was resected in 11 cases, Simpson grade Ⅲ was resected in 9 cases, Simpson Ⅳ was resected in 4 cases. All 16 cases of the outer SRMs were in accord wi th Simpson grade Ⅰ and Ⅱ, and no significant complications occurred after operat ion. In 29 cases of medial SRMs,25 were performed with Simpson Ⅰ, Ⅱ and Ⅲ resection. Among medial SRMs,3 cases were subtotally resected Simpson IV re section,because of the difficulty separation with middle cerebral artery.in 1 case attachment to oculomotor nerve after invading cavernous sinus in the other case.Conclusion:because of the complex relationship between SRMs with skull base nerve and blood vessel,even if complete stopbleeding in zhe microsurge, postoperative hematoma for Sphenoid Ridge Meningioma sometimes happen.AND easily happened in inner SRMs or major tumor patients.
