object : To Observe the efficacy and safety of bivalirudin therapy in STEMI patients undergoing Emergency PCImethods : Randomly selected 80 STEMI patients with emergency PCI from August 2014, to Feb 2015 in Dalian Central Hospital,40 cases with bivalirudin therapy and 40 patients were treated with unfractionated heparin(UFH). At the end of PCI, TIMI flow grade and myocardial perfusion grade(TMPG) were observed in the two groups. ST-segment, LVEF, left ventricular end diastolic diameter(LVEDD) and the incidence of major adverse cardiac event(MACE) and bleeding and the incidence of thrombocytopenia after 30 days were compared between two groups.Results : No statistically significant in interventional treatment path,utilization ratio of thrombus suction device; TIMI flow grade, TMPG a( P>0.05),but there was in utilization ratio of tirofiban 、 ST-segment,30 d LVEF, LVEDD(P<0.05). No deaths and severe bleeding in 2groups.There was not difference in re-hospitalization rate, incidence of heart failure or other adverse reaction between 2 groups( P>0.05).Bivalirudin group had higher gingival bleeding and hematuria(P<0.05). The two groups(1 case of stent thrombosis, 1case of heparin induced thrombocytopenia in unfractionated heparin group.Conclusions:In STEMI patients with emergency PCI application bivalirudin is of good safety and effect, bivalirudin can be used as of emergency PCI in patients with STEMI conventional anticoagulant treatment measures.
