The Relation Between Mean Platelet Value And Coronary Bifurcation Lesion In Patients With Acute Coronary Syndrome

Abstract/Summary:
Objective: To explore Syntax score( SS), bifurcation lesion score( BLS) and bifurcation lesion score /syntax score( BLS/SS) in patients with different mean platelet volume( MPV), and analyze the relationship among MPV, SS, BLS and BLS/SS.Methods: This was a retrospective study, and ultimately included 747 patients with acute coronary syndrome( ACS). MPV, creatine kinase isoenzyme( CK-MB), and troponin I( CTNI) were detected by using blood samples collected right at the time of admission. Fasting blood samples within 24 hours were available for other biological indicators. Patients’ general clinical characteristics such as age, sex, smoking history,heart disease history, hypertension, diabetes mellitus( DM) history and neoplasm history were made detailed records. SS and BLS were calculated based on the Syntax Scoring System, and then BLS/SS was computed. We compared BLS and BLS/SS of high MPV group( MPV≥9.3fl) to low MPV group( MPV<9.3fl), and analyzed the differences of MPV and SS in patients with and without bifurcation lesion( BL).Pearson correlation analysis was conducted to estimate the association among MPV, SS and SS/BLS. Finally, we used multiple linear regression to evaluate whether MPV was correlated to SS, BLS or BLS/SS with existence of other confounding factors.Results: SS of high MPV group was significantly higher than that of low MPV group( 19.03±9.58 vs 16.88±9.30, p<0.05), so as BLS, BLS/SS and the amount of BL( BLS: 11.16±8.12 vs 9.43±7.53, p<0.05; BLS/SS: 52.93±23.02 vs 45.94±24.24.02,p<0.05; amount of BL: 1.89±1.09 vs 1.66±1.06, p<0.05). Besides, for patients with BL,their MPV and SS were both higher than those without BL( MPV: 9.526±1.301 vs9.035±0.848, p<0.05; SS: 18.556±8.865 vs 5.616±5.663, p<0.05). Pearson analysis illustrated that SS, BLS and BLS/SS were positively correlated to MPV( SS: r=0.158,p<0.05; BLS: r=0.178, p<0.05; BLS/SS: r=0.251,p<0.05). SS still associated with age( r=0.118, p<0.05), hypertension( r=0.097, p<0.05), DM( r=0.114, p<0.05) and fasting blood glucose( FBG; r=0.092, p<0.05). BLS was also correlated to age( r=0.181, p<0.05), hypertension( r=0.077, p<0.05) and DM( r=0.088, p<0.05).BLS/SS still associated with age( r=0.168, p<0.05). Multiple linear regression revealed that MPV, age and DM were finally correlated to SS and BLS with existence of other confounding factors( SS: MPV: β=0.155, p<0.05; age:β=0.126, p<0.05; DM: β=0.108,p<0.05; BLS: MPV:β=0.181, p<0.05; age:β=0.190, p<0.05; DM: β=0.085, p<0.05),MPV and age still associated with BLS/SS with existence of other confounding factors( MPV: β=0.260, p<0.05; age: β=0.181, p<0.05).Conclusion:1. MPV was positively related to the severity of total coronary lesion in patients with ACS. And MPV somehow had predictive value.2. MPV positively associated with the severity of coronary bifurcation lesion in patients with ACS. And MPV somehow had predictive value.3. MPV was positively related to BLS/SS in patients with ACS. And MPV somehow had predictive value.
