Currently sepsis and septic shock has become the most important reason of noncardiac intensive care unit mortality. The clinical features of sepsis mainly for SIRS(Systemic Inflammatory Response Syndrome) and MODS(Multiple Organ Dysfunction Syndrome). In the pathophysiology of sepsis, leading to endothelial injury is an important mechanism of SIRS and MODS. Endothelial cell apoptosis in an activated state or releasing tiny membrane pellets, i.e., endothelial cell particles(endothelial microparticles, EMPs), changes EMPs level is considered as the evaluation of endothelial dysfunction markers, and endothelial function at the same time closely related to the development and progression of disorders involving sepsis role of EMPs. Alprostadil has a dilation of blood vessels, inhibiting platelet drama, protection, anti-inflammatory, microcirculation, as well as a role in protecting endothelial cells. Are alprostadil improvement from sepsis, affect the level of EMPs to be studied. In this study, cecal ligation and puncture(CPL) rat model of sepsis, given different doses of alprostadil, dynamic observed in rat plasma endothelial microparticles(EMPs) level, through changes in endothelial cells in rats with sepsis cases particles and trends, investigate whether alprostadil improve sepsis inflammatory factors, the impact on the level of EMPs, and EMPs levels and the severity of sepsis relevance for clinical use alprostadil treatment of sepsis and analysis of endothelial function obstacles, and to provide a new reference to elucidate the pathogenesis of sepsis, prognosis and evaluated.Methods: 180g-220 g 100 SD male rats were randomly divided into five groups, namely sham operation group(Sham group), cecal ligation and puncture group(CLP group), cecal ligation and puncture alprostadil administered low-dose group(CLP + PGE1- a group), cecal ligation puncture alprostadil dose treatment group(CLP + PGE1-2 group), cecal ligation and puncture alprostadil high dose treatment group(CLP + PGE1-3 group). Established sepsis model by cecal ligation and puncture(Cecal ligation and puncture, CLP), CLP + PGE1-1 group, CLP + PGE1-2 group, CLP + 30 minutes before surgery PGE1-3 group respectively alprostadil(PGE1 2ml-10ug) injection, 10 ml of physiological saline was added, according to low, medium and high dose respectively 1.5ug, 3.0ug, 6.0ug intraperitoneal injection, and 25 min in surgery were injected 0.5ug, 1.0ug, 2.0ug, each The total dose was 0.25,0.5,1.0ug / kg. Sham group(Sham group) and cecal ligation and puncture group(CLP group) were injected with a low dose of the same quality of saline. Each group respectively depicting six hours after surgery 0,6,12,24 rats abdominal aortic blood, separating plasma detection EMPs percentage concentration of TNF-α, using SPSS16.0 software package for data of two sample t test to detect the number, P <0.05 as statistically significant difference was observed in rats in each group and the general state of the application H & E staining of renal histological changes.Results: 0 h CLP group showed no Sham group concentrated serum TNF-α levels significant difference(P> 0.05), 6h, 12 h, 24 h CLP Sham group and between TNF-α levels have significantly increased, indicating that there is significant difference(P <0.05). 0h CLP + showed no significant difference between serum TNF-α concentration PGE1 group and Sham group. Comparison between groups 6h CLP + PGE1 group and Sham group, 12 h, 24 h serum TNF-a increased, suggesting that there was difference of(P <0.05). 0h CLP + no serum TNF-α concentrations between PGE1 group and CLP group was statistically significant(P> 0.05). At 6h, 12 h and 24 h serum TNF-α concentration is relatively, CLP + PGE1 concentration of serum TNF-α levels were significantly lower compared with CLP, there are significant difference(P <0.05). Different doses of alprostadil group, between the same point in time the serum TNF-α concentrations tended to increase with increasing dose decreased. No comparison between 0h CLP group and Sham group EMPs significant difference(P> 0.05), 6h, 12 h, between the group and the Sham EMPs 24 h CLP level was significantly increased, a significant difference(P <0.05). No comparison between 0h CLP + PGE1 group and Sham group EMPs significant difference. CLP + between PGE1 group and Sham group 6h, 12 h, 24 hEMPs have increased, there is a statistically significant difference(P <0.05). 0h CLP + free between PGE1 group and CLP group EMPs significant difference(P> 0.05). At 6h, 12 h and 24 hEMPs comparison, CLP + PGE1 group EMPs levels significantly lower than the CLP group, statistically significant differences(P <0.05). Different doses of alprostadil group, between the same point in time with the serum levels of EMPs dose increase tended to increase gradually decreased. Septic rats 6h, 12 h, 24 h condition gradually worsened, acute kidney injury worse. CLP rats with normal renal structure was destroyed. CLP + degree of renal impairment PGE1 group were reduced in rat liver tissue microscope alprostadil high dose group showed a small amount of glomerular fibrosis, accompanied by a small number of inflammatory cell infiltration, mainly lymphocytes, renal ball swelling, but the degree of cell swelling, necrosis area reduced.Conclusion: Aggravate the disease in rats with sepsis, TNF-α concentrations in septic rats gradually increased, EMPs levels will be increased, with sicker when prompted sepsis endothelial injury also increased. In the application of alprostadil treatment Sepsis, use alprostadil group EMPs levels were lower than those without the application, it may prove alprostadil protect endothelial function, reduce inflammation injury.
