Objective: To investigate the etiology, clinical features and impact factors which is related to prognosis of acute kidney injury(AKI).Methods: There hundred and twenty patients with no previous kidney disease but diagnosed acute kidney injury were admitted in EICU of the first affiliated hospital of the Dalian Medical University between 2010 and 2014.Recording the SOFA score in 24 hours, name, sex, age, diagnosis, infection, days of living EICU, surgery,hypertension,the way and time of hypertension, AKI RIFLE(first diagnosed),AKI RIFLE(leave hospital),urine protein(first diagnosed), urine protein(leave hospital)outcomes.320 patients with AKI were divided into the young group(≤40 years old),middle-age group(41~60 years old), elderly group(≥61 years old),according their ages; sepsis group and non-sepsis group according their etiology; survivor group and death group according their outcomes. the etiology, clinical features and impact factors which is related to prognosis were analyzed.Results : There were 201 males( 62.8%) and 119 females( 35.2%) with an average age of 61.85 ±18.49 years. The common causes were sepsis acute intoxication,cardiovascular disease; the elderly people more easier to be attacked by AKI; Sepsis and acute intoxication has significant difference between different age groups(P<0.05),the sepsis of elderly group was higher than the other age groups; but acute intoxication was lower than the other groups. There was significant difference in Surgery,hypertension,and mortality between different age groups(P<0.05).The elderly group was higher thanthe other age groups,however,there was no difference between middle and young-age groups. There was significant difference of age, mechanical ventilation, hypertension,proteinuira, and mortality in sepsis group and non-sepsis group. There was significant difference of age, sofa score, surgery, mechanical ventilation, proteinuira and sepsis in survivor group and death group. Logistic regression showed that older age, sofa score,surgery were the risk factors of poor prognosis and mechanical ventilation was the protective factor in acute kidney injury patients.Conclusions : The results indicate that sepsis was the significant etiology to lead to acute kidney injury. older age more easily to be attacked by acute kidney injury.When sepsis lead to AKI, it has higher mortality and poorer prognosis. Elderly age, SOFA score and surgery were risk factors and mechanical ventilation was protective factor for patients with acute kidney injury.
