Objective:To analyze the efficacy and the prognosis of radical transurethral therapy for muscle invasive bladder cancer and to provide new ideas for the treatment of muscle invasive bladder cancer.Methods:A retrospective analysis of73patients in our hospital from January2011to October2014, the patients underwent radical resection of the muscle invasive bladder cancer, and the pathology suggested muscle-invasive urothelial carcinoma. There are62males and11females, aged between46to89years with the average age of72years. Radical tumor resection range will be from the tumor to outer wall of the fat layer, around1-2cm tumor at the base of normal bladder mucosa [1] by transurethral cystoscopy.8cases underwent postoperative chemotherapy,53routine intravesical chemotherapy,4patients did not undergo the treatment, eight patients were lost to follow.Results:The73operations of radical transurethral bladder goes well, average operation time is50mins; average volume of bleeding27ml, intraoperative complications were not occurred, the average length of stay is5.8days. Followed up65cases,8cases underwent postoperative chemotherapy,53underwent routine intravesical chemotherapy, four cases did not undergo further treatment, in1year12cases found out postoperative recurrence, the recurrence rate of18.5%, at the end of the follow-up period,23cases appeared recurrence, the recurrence rate is35.4%, recurrence time is from2-34months, after the relapse16cases underwent transurethral tumor resection, one case underwent partial cystectomy, four cases underwent radical cystectomy.56cases Survived and9cases died, including4cases of bladder cancer recurrence or metastasis. Eight cases underwent postoperative systemic chemotherapy, one found recurrence after surgery in11months,he underwent radical cystectomy surgery, other seven cases had no recurrence, living at the end of follow-up, survival time is between5-35months.53cases underwent intravesical chemotherapy,20cases had recurrence, the recurrence rate is37.7%,31cases were followed up for more than2years, the recurrence rate of64.5%(20/31), the median time of relapse is19.44months.Conclusion:For patients with muscle invasive bladder cancer, radical transurethral resection is a optional treatment, postoperative intravesical chemotherapy could lead a high recurrence rate; adjuvant systemic chemotherapy can achieve better treatment.
