Objective: To retrospective study discontinuous and continuous completeness of androgen deprivation therapy in patients with advanced prostate cancer clinical efficacy and impact on patients quality of life.Methods: From January 2009 to December 2012 in our hospital line bilateral orchiectomy of 70 cases of advanced prostate cancer patients as the research object, According to the different follow-up treatment will be divided into intermittent androgen depriation therapy group(IAD) and completeness androgen deprivation therapy group(CAD), including IAD group of 32 cases, 38 cases of CAD group.Through regular review and follow-up, record the change of the two groups of patients with blood PSA, the hormone dependency prostate cancer occurrence, and uses the quality of life scale EORTC QLQ C30 and QLQ PR25 for patients survival quality data,by the two groups after treatment in patients with the clinical results, complications, and the quality of life for statistical analysis.Results:1、IAD group of 32 patients were followed up for 12 ~ 38 months, an average follow-up of 33.10±9.8 months. CAD group of 38 patients were followed up for 10 to 37 months, an average follow-up of 32.5±9.5 months. In CAD group 2 patients in the late follow-up serum PSA > 30 ng/ml, combined with patient imaging data and serum testosterone was confirmed patients progress to the hormone dependency prostate cancer(AIPC); IAD group in the late follow-up serum PSA > 30 ng/ml, combined with imaging data and progress of tests confirmed patients serum testosterone is a hormone dependency prostate cancer(AIPC); Although the IAD group is lower than the CAD on the incidence of disease progression, but two groups by chi-square disease progression without statistical significance, 2、IAD group and CAD group after treatment after treatment 1, 3, 6, 9 and 12 months PSA decline in horizontal contrast has no statistical significance(P < 0.05). 3、IAD group compared with CAD group of adverse reactions after treatment, two groups compare the differences in the total incidence of adverse reactions are significant(P < 0.01), with statistical significance. In addition to the incidence of gastrointestinal symptoms, including diarrhea, nausea, vomiting, no statistical significance, two groups were statistically significant compared incidence of hot flashes, headache(P = 0.012, 0.039, less than 0.05). 4、Two way patients 1 year after treatment with quality of life scale EORTC QLQ C30 and QLQ PR25 rate.Using EORTC QLQ C30- scale survey can be found that IAD group and CAD group in physical function(or function) the impact of P = 0.013(P < 0.05), the difference was statistically significant.We can find that in the other functional assessment, two groups of patients in the role function, emotional function, social function and the overall quality of life of IAD group scores are relatively high, this represents a relatively high quality of life, but there was no statistically significant difference compared with CAD group(P > 0.05).QLQ- PR25 scale contrast can be found in prostate cancer drug treatment side effects, IAD group scores are relatively low, compared with the CAD group difference was statistically significant(P < 0.05), Finally through sexual function indicators we can, we found that the patients after surgical castration whether using IAD treatment or CAD, its functions are affected by large, the vast majority of patients with loss of sexual function.Conclusions:Line for patients with advanced prostate cancer after bilateral orchiectomy, intermittent androgen depriation therapy can effectively control the development of cancer, and in the treatment of adverse reactions and the influence of the quality of life is better than continuous androgen deprivation, is worth in clinical use and promotion.
