Objective:Discussion on type 2 diabetes mellitus (T2DM) blood glue ose change characteristics of the dynamic regularity of blood glucose c hange and correlation of different TCM Syndrome Typesand some speci al T2DM patients, classified as syndrome differentiation of T2DM can p rovide the objective basis of reference value for the application of TC M theory, control of blood glucose fluctuation and slow down the proc ess of T2DM and chronic complications develop accurate point. In order to explore both with the traditional Chinese medicine theory and meet the new idea of T2DM in clinical diagnosis and treatment provide a r eference to the actual needs, hopes for T2DM in the etiology of Chines e medicine, prevention and treatment of clinical science and other aspect s of significance.Methods:According to diagnosis standard by diabetes branch of the Chinese Medical Association proposed 2010 Edition "China type 2 diab etes prevention guide" in T2DM and the Chinese society of Chinese med icine prepared 2007 version of "Chinese medicine" the guidelines for pr evention and treatment of diabetes in the diabetes disease classificatio n, formulation into,exit criteria, exclusion criteria, will be incorporated into the clinical data of 60 cases of subjects the dynamic blood glucos e fluctuation, including related index, glycosylated hemoglobin,gender, a ge, course of disease,, body mass index (BMI), waist hip ratio (WHR), b lood lipid,evaluate the history, complications, analyses the inner connec tion between these indexes and TCM syndrome types, especially focusin g on the relevance analysis of dynamic blood glucose fluctuation evalu ation index and traditional Chinese medicine syndrome type.Results: ①The three group of comparison subjects history:the sputu m (wet) the incidence of heat up is deficiency of both qi and Yin grou p mutual hypertension rate is high (P< 0.05), the other groups were tes ted had no obvious difference between the basic medical history (P> 0. 05);②The three group comparison subjects laboratory indicators:three gro ups of subjects with high density lipoprotein, triglyceride, fasting blood glucose, glycosylated hemoglobin vein showed no statistically significa nt difference (P> 0.05), sputum (wet) heat up total cholesterol and low density lipoprotein were significantly higher than the other two group s (p<0.01);③Through comparing three groups of subjects with chronic complica tions:three groups of subjects in heat injury Tianjin groups of patients with diabetic retinopathy were significantly more than sputum (wet) and heat up (all P<0.05); sputum (wet) and heat up in patients with diabetic neuropathy patients than heat Sheng Jin injury group; three subjects the risk of diabetic lower extremity vascular disease in comparison, sputu m (wet) and heat up heat Sheng Jin group> injury of deficiency of both qi and Yin group;the three group of subjects with diabetes kidney dise ase in disguised form comparison,deficiency of both qi and Ying rou p, sputum (wet)and heat up heat Sheng Jin group> injury;④The blood glucose fluctuation trend:the average blood glucose of s ubjects value within a day about 1/4 time at a high level (>11.lmmol/ L); the night around 3Am and 24h patients blood glucose before three m eals is relatively low period; the three groups of patients with differen t blood glucose peak time, heat Sheng Jin group than in the meal injur y after 1 hours to reach the peak, the other two groups at 2 hour after the meal to the peak;⑤The three group of 24h average blood glucose compared to each ot her, sputum (wet)and heat up heat Sheng Jin group> injury of deficiency of both qi and Yin group;⑥Control of blood glucose levels between the three groups had signi ficant difference and standard deviation, sputum (wet) and heat up hea t Sheng Jin group> injury of deficiency of both qi and Yin group (p<0. 01). The three group of mean amplitude of glycemic excursion, the ma ximum amplitude of glycemic variability, days drift times, after breakfas t blood glucose fluctuation amplitude, after dinner blood glucose fluctua tionamplitude and compared, heat Sheng Jin injury group, sputum (wet) an d heat up>deficiency of both qi and Yin group (P<0.05);⑦High blood glucose levels in the three groups, the percentage of ti me compared to theday, sputum (wet) and heat up higher than the other two groups, and compared with the deficiency of both qi and Yin grou p had significant difference (p<0.01); hypoglycemia group three percenta ge compared to day time, heat Sheng Jin injury group, deficiency of both qi and Yin group, sputum (wet) heat each groups (p<0.01);⑧In this study, the CGMS system of the measured value of interstiti al fluid glucose and venous blood glucose values were positively relate d to;⑨That because the observation time and sample size limitations, dyna mic blood glucose change characteristics of some special groups is not found, the contrast and temporarily out of 2 days between glucose condit ion.Conclusion: ①The average blood glucose of subjects value within a day about 1/4 time at a high level(>11.lmmol/L). About 3Am and 24h p atients blood glucose before three meals is relatively low period. Explai n the fluctuation of dynamic glucose monitoring can monitor blood glu cose in patients with more comprehensive;②The three groups of blood glucose level of the standard deviation t o have the obvious difference, and sputum (wet) and heat up heat Shen g Jin group> injury of deficiency of both qi and Yin group (p<0.01); ex plain the fluctuations in the T2DM dynamic blood sugar related with th e syndrome type of traditional Chinese medicine, one of the objective indexes of dynamic blood glucose monitoring can be used as the syndr omes of traditional Chinese medicine.③The above conclusions provide the basis for clinical trials of a ne w idea to reduce the T2DM of traditional Chinese medicine in the trea tment of blood glucose fluctuation based on.
