Sanqi Panax Notoginseng Injection On The Inflammatory Response In Patients With Acute ST Segment Elevation Myocardial Infarction And The Influence Of Cardiac Function

Objective:It aims to observe the inflammatory reaction and cardiac function effect of myocardial infarction patients with acute ST segment elevation by using Sanqi Panax Notoginseng injection.Methods:We collected 70 cases of patients who hospitalized in the first affiliated hospital of Henan University of TCM (Traditional Chinese Medicine). All of them were diagnosed that suffering from the acute ST segment elevation myocardial infarction, blood stasis syndrome (based on the traditional Chinese medicine diagnosis), and emergency treatment by using PCI surgery. Among them, 34 people were divided into experimental group, and 36 people belong to control group. Two groups of patients were accepted the therapeutic schedule in accordance with the latest PCI treatment guidelines. All of them regularly took aspirin 300 mg chewing and clopidogrel 600 mg orally.People of experimental group were given Sanqi Panax Notoginseng Injection 400 mg plug push in vein before operation and continuous intravenous drip after,1 times a day and lasts for 14days, while patients in the control group were given the same amount of a placebo. Compared analysis of the two groups of patients will be done before and after operation, including the level of hypersensitive c-reactive protein (HsCRP), five protein 3 (PTX-3), myocardial enzymes (Ck-MB), allergic troponin (cTnT), the different levels of brain natriuretic peptide (BNP), differences of the left ventricular ejection fraction (LVEF) and the left ventricular end-diastolic (LVEDd) after the operation and one month later, and the change of blood stasis syndrome score.Results:1、The comparison of baseline of two groupsThe baseline data of the two groups of patients including gender, ase hypertension history, history of diabetes, blood lipid metabolic abnormalities history, history of myocardial infarction, myocardial infarction, cumulative lesions are no statistical difference (P>0.05).2、The changes of inflammation indexComparison of the levels of serum HsCRP and PTX-3 of the two groups of patients before treatment shows that there is no statistical significant difference (P> 0.05); but after the treatment, the levels of both are lower, and the differences of the control group are no statistical significance before and after treatment (P>0.05). Compare with the control group, the data of experimental group has a remarkable reduction and the difference has statistical significance [HsCRP:(2.48±1.76) VS (5.12±1.54)mg/L,t=3.264,P=0.024;PTX-3:(4.8± 1.3)VS(7.2±2.3)ng/ml, t=3.526, P=0.01].3、The changes of myocardial injury markersComparison of the levels of CK-MB and cTnT of the two groups of patients before treatment shows that there is no statistical significant difference (P>0.05); after the treatment, the levels of two groups significantly fall down, and the experimental group shows obvious lower than the control group, thus has the statistical significance [CK-MB:(62.4±12.3)VS (142.6± 36.4) U/L, t=4.231 P=0.016; cTnT:(12.5± 3.6) VS (34.8± 7.2)μ g/L, t=3.957, P=0.014].4、The BNP evaluation resultsThere is no statistical significant difference (P>0.05) about the comparison of BNP levels of the two groups once they hospitalized; however, the level is apparently lower within 15days after their operation, and the experimental group is obvious lower than the control group. The difference has statistical significance (153±122) VS (249±191) pg/ml, t=3.426,P=0.026].5、Heart Doppler ultrasoundTwo groups of patients receive immediate comparison of heart doppler ultrasound LVDd and EF level, there is no statistical significant difference (P 0.05); within 15days and 30days after the operation, the level of LVDd are significantly lower while the EF level rises. As time goes, the situation improves and the comparison has statistical significance (P<0.05). There is no statistically significant difference (P>0.05) about the comparison of LVDd and EF levels of the two groups within 15days after their operation, but within 30days of postoperation, the level of LVDd of experimental group decreases more than control group, and the EF level rises as well. There are statistical significance about their differences (LVDd (53.6±4.8)VS(54.7±3.9)mm,t=1.925, P=0.041; EF:(50.3±4.1) VS (49.6±3.8)%, t=1.892, P=0.043).6、Comparison of blood stasis syndrome scoreThere is no statistically significant difference (P>0.05) of the two groups on their blood stasis syndrome score before operation; within 15 days after operation, the blood stasis syndrome score were significantly lower, and the experimental group decrease more than the control group, also their differences has statistical significance.(28.6±10.1) VS (34.1±11.2) points, t=2.856, P=0.039).Conclusion:Sanqi Panax Notoginseng Injection can reduce the inflammatory reaction of patients with acute ST segment elevation and myocardial infarction, reduce cardiac function and increase the integral of blood stasis syndrome.
