Objective: Multi- inspection system of cardiovascular disease is put forward and constructed in this paper. The Multi- inspection system aims at making contributions to the combination of Chinese medicine inspection with Chinese medicine informatization. The Multi- inspection information of unstable angina pectoris(UAP) is extracted and verified primarily, which provides clinical data to construct the perfect Multi- inspection system of cardiovascular disease and explores reasonable verification method.Methods:1. Construct the Multi- inspection system of cardiovascular disease based on the combination of Chinese medicine inspection with Chinese medicine informatization.2.(1)Extract the Multi- inspection information of unstable angina pectoris(UAP) and make the clinic research form through literature research and summarization of clinical experience, based on diagnoses of Chinese medicine.(2) 207 cases that meet the standard of case group and control group are observed clinically, taken photos and filled in the clinic research form. The information of 207 cases is analysed through chi-square test and binary logistic regression analyses. Make the observation target of the Multi- inspection system of UAP base on the analysis results.(3) Reanalyse the 207 cases according to the above observation target and calculate sensitivity, specificity, misdiagnosis rate and omission diagnostic rate of the observation target. Calculate the correlation and consistency between the observation target of the Multi- inspection system of UAP and the standard diagnosis to verify the observation target of the Multi- inspection system of UAP preliminarily.Results:1. Put forward the specific content of constructing he Multi- inspection system of cardiovascular disease.2. Extract and verify the Multi-inspection information of unstable angina pectoris(1) 27 Multi-inspection clauses of unstable angina pectoris through literature research and summarization of clinical experience.(2) 16 Multi-inspection clauses of unstable angina pectoris that P below 0.5 through chi-square test and fisher’s exact test(3) 12 Multi-inspection clauses of unstable angina pectoris that the correlation is strong and divided into four groups according to the crude OR. And the above four groups are given scores respectively to be easy to the integral statistics. The results are as follows. Give three points to the first kind((1)information of nails;(2)obvious veins in thenar;(3)texture of “*†at the end of the lifeline); Give two points to the second kind((1)red or purple palm;(2)deep and horizontal grain at the nose root;(3)texture of “+†in the heaven area); Give one point to the third kind((1)texture of “*†under the forefinger;(2)dark red spots in thenar;(3)bolted ditch;(4)information of A third on the forehead);Give 0.5 points to the fourth kind(((1)texture of “*†at the end of the head line;(2)short fingers or ridgy finger pulp, bulky fingertips shaped as drumstick or gecko)(4)Part of verification: Set 10 points as the limit and the integral results are positive(+)when the integral results are greater than or equal to 10 points and other results are negative(-).The sensitivity is 0.847 and the specificity is 0. 708. The sum of above two data is the largest. The omission diagnostic rate is 0.153 and the misdiagnosis rate is 0.292. The sum of above two data is the least. At the same time, the Youden index is 0.555 and the truth rate is 0.783. Both of them are the largest. So patients have higher probability to suffer from unstable angina pectoris when the the integral results are greater than or equal to 10 points.Conclusion:1. During the construction of the Multi- inspection system of cardiovascular disease, the Chinese medicine inspection contents about cardiovascular disease are collected and generalize systematically. The Multi- inspection system combines with data mining, software design and teleconferencing technology and helps to explore Chinese medicine inspection theory. 2. The observation target of the Multi-inspection system of can be the assistant diagnostic method and contribute to set the Chinese medicine inspection standard. |
