Background:Developmental Dysplasia of the Hip (DDH) is a common adult hip joint disease, total hip arthroplasty can get satisfcatory result as the final treatment of the disease. It often cause prosthesis acetabular cup coverage because of acetabulum bone defect of different level In the operation. Bulk femoral head or iliac bone grafting technology for acetabula reconstruction can get satisfactory coverage and stability of the prosthesis so as to prolong the life of the prosthesis, improve the patient satisfaction degree.Objective:The purpose of this study is to explore the application of bone grafting technology (structural and the particles bone graft) in THA of DDH and evaluate the result with imaging changes and follow-up observation, postoperative hip function scoreand to test and study influence of operational result from bone graft coverage and reconstructive level.Methods:38 patients with development hip dysplasia were performed with structural or the particles bone graft for actabular reconstruction in THA during March 2013 to October 2014 in the Department of joint of Shandong University of traditional Chinese Medicine First Affiliated Hospital (Shandong Province Hospital of traditional Chinese Medicine). 40 hips in 35 patients were successfully followed up in average ofl.5 years。According to Crowe rating,21 hips were stage II,13 hips were stage Ⅲ with 6 hips as stage IV. The deficient acetabula were reconstructed by bulk femoral head autograft. With bone coverageguaranteed, patients had prothesis installed at original or closed level of true acetabulum. Radiographic changes and Harris scores were evaluated.Results:There were 35 patients 40 hips were successfully followed up except for 1 cases of single hip surgery for dislocation revision and 2 patients failed to follow up. there was no prosthesis loosening and bone graft loosening. The image changes of bone graft and host bone fusion, contact surface blur, bone trabecular reconstruction, no implant displacement and bone resorption. The preoperative Harris scores averaged 44.99 points, six months after operation, the average Harris score was 89.53, significantly higher than before (PS≦0.01). The effect of the Harris score was 100%.Conclusion:Postoperative follow-up of patients with hip joint is a X ray film visible part of the bone graft with the host bone fusion, contact surface with time gradually blurred even disappeared, and obviously improve the patients postoperative Harris score compared with the preoperative patients with hip joint function improved significantly, improve the life quality of patients. That artificial total hip replacement therapy used in the process of dysplasia of the hip surgery treatment acetabulum bone graft technology to the function of postoperative patients with hip bone defect is a safe and effective in the short term.
