Application of minimally invasive technique is in progress and the development of surgical operation. Laparoscopic as a representative of minimally invasive operation has been widely used in clinical treatment, the advantages of small trauma, less bleeding, faster recovery have been fully reflected in the rectal carcinoma radical operation. Da Vinci robots surgical system was the product of laparoscopic technological development. Application of the Da Vinci robot operation system in the field of surgery minimally invasive advantages will raise to a new height. There are a number of scholars and various reports demonstrated that Da Vinci surgical robot system was in the use of the feasibility and effectiveness of surgical operation. In the rectal carcinoma radical operation especially low rectal cancer operation, Da Vinci surgical robot system reflects more fully the technical advantages that were the anatomy in the narrow pelvic tissue, separation of mesangium vessel, identification of nervous plexus, manual suture and other aspects.Objective: By analysis on the two cases of radical resection of low rectal cancer that were applied between the Da Vinci robot operation system and laparoscopic assisted, in order to understand the therapeutic effect and differences in radical operation of technology for low rectal cancer between the Da Vinci robot operation system and laparoscopy operation.methods: 40 cases of low rectal cancer using the Da Vinci robot assisted operation system were collected from the General Hospital of Shenyang military region since 2013, 2 to 2015, 2, and select the same number of radical resection in laparoscopic assisted rectal cancer cases. It was analysis about the recovery time of operation, bleeding, postoperative(including the postoperative recovery of gastrointestinal function, the resumption of normal diet time, catheter removal time, operation complications) and pathological examination in the two groups respectively.Result: The operations of the selected patients were completed, operation time of the robot group is longer than the laparoscopic group(t=1.95, P=0.06), but the difference was not statistically significant(P>0.05). The amount of intraoperative bleeding of robot group were less than the laparoscopic group(t=4.72, P=0.00003), the difference was statistically significant(P < 0.05), intraoperative bleeding was significantly reduced in the robot group. postoperative recovery of gastrointestinal function and diet were similar in the two groups, but there are differences in catheter time( t = 2.20, P = 0.034), the difference was statistically significant(P <0.05).postoperative extubation time in robot group was earlier than the laparoscopic group, the postoperative urinary retention rate is low, the influence of operation on patients with urination function was small, micturition function recovered rapidly, and for the postoperative patients with sexual dysfunction(erectile dysfunction ED), the incidence of robot group(15.4%) is less than the laparoscopic group(21.4%). Pathological examinations showed that the two groups reached the radical operation of rectal cancer operation standard requirements, including mesenteric integrity, cleaning of the mesenteric lymph nodes, number of resected bowel resection margin tissue of cancer tissue. two groups of lymph node number(t=-3.85, P=0.0004) are different(P>0.05), the robot group is obviously higher than the laparoscopic group. All the robot operation excision of the mesorectum were well, then two cases of laparoscopic operation of mesorectum integrity was destroyed.Conclusion: The Da Vinci robot operation system and laparoscopic assisted is as safe feasible and effective in the radical operation of rectal cancer application, fully meet the tumor treatment principle, the robot operation in the cleaning robot mesenteric and pelvic lymph node was more thoroughly, operation cure degree and the pelvic autonomic nerve identification and protection is higher, operation injury was smaller, it can reflect the minimally invasive surgery to reduce the damage control operation requirements. Da Vinci robot operation system is better than traditional laparoscopic technique in low rectal cancer operation, the prospect of application is wider.
