PURPOSE:To investigate the therapeutic effects of 894 coblation-assisted treatment in adult with obstructive sleep apnea-hypopnea syndrome,including the quality of life after operation,complications and recurrence.OBJECTS:The 894 OSAHS patiens who came to our depatment complaiming sleep snoring during February 2012 to December 2013, 833 cases of male and 61 cases of female, aged 34 to 68 years, and the average age was 51. Patients associated with other diseases affecting social features and disabilities are not included in the studyMETHODS:A review of 894 coblation-assisted treatment in adult with obstructive sleep apnea-hypopnea syndrome was presented,. Patients with polysomnography sleep apnea-hypopnea index (AHI)≥5 times/h and obstructive respiratory events mainly were diagnosed OSAHS according to the 2009 OSAHS diagnostic criteria enacted by the Chinese Medical Association of Otorhinolaryngology Branch. We located the patient’s airway obstruction positioning according to the clinical and imaging, fiber endoscopy examination and choose different surgical procedures.During the 894 OSAHS patients,14 cases did a deviated septum surgery,27 cases did inferior turbinate radiofrequency ablation and deviated septum surgery,5 cases did adenoid ablation surgery,262 cases did Ca-uvulopalatopharyngoplasty surgery,191 cases did coblation- channeling for the tongue and Ca-uvulopalatopharyngoplasty surgery,64 cases did tongue tonsil ablation, Ca-UPPP and CCT surgery,234 cases did inferior turbinate radiofrequency ablation, Ca-UPPP and CCT surgery,97cases did inferior turbinate radiofrequency ablation, deviated septum surgery, Ca-UPPP and CCT surgery.And duiring the 848 cases who taken Ca-UPPP surgery,383 cases did front soft palate surgery.and 465 cases did partial resection of the soft palate surgery. Summary analysis of its clinical efficacy, complications, quality of life scores and postoperative recurrence.RESULTS:After operation 6 months, the sleep apnea-hypopnea index, the lowest oxygen, the average oxygen were improved remarkably (P<0.01)and the symptoms of snore and choke improved. SF-36 health questionnaire survey showed that social function, energy and mental health dimension scores were significantly higher than the preoperative(P< 0.05), other dimensions scores relatively significant difference does not appear. There were 18 patients occurred Secondarily bleeding within 2 weeks.the bleeding rate of 2.01%.8 cases of postoperative bleeding occurred 1-2 days after operration,and 10 cases of postoperative bleeding occurred 10-14 days after operration.13 cases of bleeding sited in the tonsillar fossa,and 5 cases of bleeding sited in the soft palate.23 patients recrudesced within 1 year, the main reason is the tongue hypertrophy or soft palate relaxation which caused by obesity. So weight control is necessary to prevent recurrence.CONCLUSIONS:Radio frequency coblation is an applicable method of therapy for patients with obstructive sleep apnea-hypopnea. It is cool and set its ablation, cutting, washing, attracting and hemostasis and other functions in one. Radio frequency coblation shows irreplaceable advantages In ENT "soft tissue site" diseases treated by surgical. The treatments differ with the obstructive location and character of upper airway.
