Aim:To investigate the relationship between posterior cerebral artery ischemic stroke and cognitive impairment.Method:This study included patients with acute posterior circulation ischemic stroke.Magnetic resonance imaging based posterior circulation infarction scale was used to calculate patients’infarction score. Mini-Mental State Examination(MMSE), Alzheimer’s Disease Rating Scale cognitive subscales(ADAS-cog),Clinical Dementia Rating scale(CDR) was used to evaluate cognition in post-stroke patients. Patients were divided into three groups:normal cognitive function, vascular mild cognitive impairment and vascular dementia according to cognitive diagnosis standards.Results:Among67enrolled patients,32patients had normal cognition(47.8%),20patients had mild cognitive impairment (29.9%),15patients had vascular dementia (22.4%). Posterior circulation infarction scores ranged from0.5to2.0:37patients with0.5, mainly for occipitotemporal lobe infarction (23patients,62.2%),31patients with1.0, mainly for pontine infarction (18patients,58.1%);2patients with2.0, mainly for cerebellar and pontine infarction. Multivariate logistic regression analysis showed that, the odds ratio (OR) of cognitive impairment in patients with occipitotemporal lobe infarction increased(OR=75.89,95%CI3.92-1470.06), while in patients with pontine infarction reduced by90%(OR=0.10,95%CI0.02-0.60); the further analysis shows that, the odds ratio (OR) of mild cognitive impairment in patients with occipitotemporal lobe infarction increased further (OR=542.24,95%CI7.85-37481.44); and that of vascular dementia in patients with cerebellar infarction also increased(OR=12.49,95%CI1.03-151.58).Conclusion:In this study more than Fifty percents patients occurred cognitive impairment after posterior cerebral artery ischemic stroke. The occipitotemporal and cerebellar infarction increased the risk of suffering cognitive impairment, while the pontine infarction negatively correlated with cognitive impairment.
