Objective:To timely detect and diagnose gallbladder cancer and take positive andeffective measures by analyzing and summarizing the clinical features of thepatients with cholecystolithiasis and gallbladder cancer.To summarize, exploreand improve the methods of the treatment and prognosis of the patients withunsuspected gallbladder cancer,from founding the clinical characteristics andtreatment of patients with unsuspected gallbladder cancer after the gallstonesurgery.Methods:I retrospectively analysis both Group A-72patients with cholecystolithiasisand gallbladder cancer-and Group B-72patients with cholecystolithiasisafter cholecystectomy slected randomly-during the period from January2007to December2014in our hospital. I also retrospectively analysis theclinical data and follow-up results of19intraoperative and postoperativepatients with unsuspected gallbladder cancer.Results:All patients were at within the scope of the study of72cases patients with cholecystolithiasis and gallbladder cancer (group A) accounted for allpatients with gallbladder cancer:81.82%(72/88).In the case of groupA, women accounted for66.67%(48/72), with over55years of age accountedfor75%(54/72). Clinical manifestations:65.28%(47/72) in group A of patientswith recurrent upper abdominal pain, Skin and sclera yellow dye,significant weight loss and other symptoms,73.61%(53/72)patients of groupB, dominated by intermittent abdominal pain, nausea, vomiting, a smallamount of examination in asymptomatic cholecystolithiasis。 Imaging: theultrasonic examination of73.61%(53/72) patients of groupA reveals the gallbladder wall thick, irregular, gallbladder shrank and thefurther examination such as CT,MRI likely diagnosis as gallbladder cancer.The diameter of GD, group A gall stones diameters larger than2cm in66.67%(48/72), group B gall stones diameters larger than2cm in26.39%(19/72).Between the two groups was statistically significant.Found19cases of unexpected gallbladder carcinoma patients:By Nevin staging,3cases of I,2cases of II,14cases of III,0cases of IV and V. patients ofI period underwent cholecystectomy live for12-91months,II period only cholecystectomy in all2cases, survival after surgery for21months, patients in the phase III group,7patients withradical surgery again, only7cases had the cholecystectomy.Survival between the two groups, there was a significant difference.(P=0.012)。 Conclusion:For the patients with gallstones:Female; age>55; Imaging promotesthe-diameter-of-the stone>2cm, suggesting highpossibilities for gallbladder cancer.Further diagnosis can reduce the occurrence of unexpected gallbladdercarcinoma at an early stage to improve diagnosis, treatment and prognosis.
