The Therapeutic Effects Of Ursodeoxycholic Acid Combined Leigongtengduogan On Primary Billiary Cirrhosis

Objective: To treat with ursodeoxycholic acid single as the control,the au-thor aimes to explore the clinical curative effect of ursodeoxycholic acid combi-ned leigongtengduogan treatment in primary billiary cirrhosis.Methods: Select32cases of patients with primary billiary cirrhosis(diagn-ostic criteria is in accordance with American Association for the Study of LiverDiseases in2009guidelines for the management of the PBC)who came fromoutpatient and inpatient of hepatobiliary and pancreatic department of the Seco-nd Hospital of Jilin University between September2012and January2015,Th-ere are6male cases and26female cases among them, mean age58.9±10.6years,the patients were randomly divided into two groups,15cases in the con-ventional treatment(also called control group), based on conventional treatmentof glycyrrhizin,coenzyme complex etc.plus ursodeoxycholic acid250mg3timesa day oral treatment;17cases in the experimental group,based on conventionaltreatment of glycyrrhizin,coenzyme complex etc.plus ursodeoxycholic acid250mg3times a day;leigongtengduogan2tablets each time,3times a day oral tr-eatment,to check and record the improvement of the general status (malaise、ab-dominal distension、loss of appetite、skin itching、right upper abdominal disco-mfort) and liver function (ALT、AST、TBIL、ALP、GGT、ALB)before treatm-ent and after treatment for1weeks,2weeks,4weeks,12weeks;after treatmentfor4weeks,24weeks,12weeks, check and record the improvement of the imm-unological indicators included of immunoglobulin (IgM、IgG、IgA);complement(C3、C4);anti-nitochondrial antibody(AMA-M2).Results:1、The improvement of general status In these aspects of malaise、abdo-minal distension、loss of appetite and so on,in the whole course of treatment, t-wo groups of patients had similar remission rate, no significant difference (P>0.0 5), but in the treatment of2weeks,the symptom relief rate of right epigastricdiscomfort and pruritus in experimental group were significantly higher than th-at in control group (P<0.05)；in the treatment of4weeks and12weeks, the ri-ght epigastric discomfort、skin itching and other symptoms in two groups werebasically relieved, there were no significant difference (P>0.05) between them.2、Liver function recovery before and after treatment According to the wh-ole clinical observation, in different treatment time point of the whole treatme-nt process,the comparisons before and after the treatment of all indicators in t-he two groups were statistically significant (P <0.05);After2weeks of treatm-ent,ALT and AST of most patients in the control group and experimental grou-p were normal, while the average reduction of TBIL、ALP、GGT in experimental group were slightly higher than that in the control group (P>0.05)；in the tre-atment of4weeks, the average reduction of TBIL、ALP、GGT in the experime-ntal group were significantly higher than that in the control group, there were s-ignificant difference (P<0.05), TBIL after4weeks of treatment in the experime-ntal group is better than that in the control group,the difference was statistical-ly significant (P<0.05);after12weeks of treatment, TBIL in two groups both h-as returned to normal, while the average reduction of ALP and GGT in the exp-erimental group were significantly higher than that in the control group, the dif-ference were statistically significant (P>0.05),ALT、TBIL、ALP after12weeksof treatment in the experimental group is better than that in the control group,the difference were statistically significant(P<0.05); in the whole course of trea-tment, the average of liver albumin (ALBg/l) in the control group and experime-ntal group were rose compared with those before treatment, but the two groups showed no statistical significance(P>0.05), after12weeks of treatment,2case-s with low albumin recovered to normal.3、The improvement of immune indexes of Immunoglobulin (IgM, IgG, IgA), complement (C3, C4) and anti-nitochondrial antibody (AMA-M2) In the treatm-ent of4weeks and12weeks,the average reduction of these immune indicatorsin two groups had no significant difference(P>0.05),While after treatment for2 4weeks,IgG and IgM in the experimental group were significantly lower than th-at in the control group (P<0.05), the difference was statistically significant;in thetreatment of12weeks and24weeks,the comparison before and after the trea-tment of all the immune indicators in the experimental group are statistically s-ignificant (P<0.05),while that was not in the control group,anti-nitochondrial an-tibody(AMA-M2) showed no significant changes before and after treatment.Conclusion: Both in the improvement of clinical symptoms and the recov-ery of liver biochemical indicators,Ursodeoxycholic acid combined leigongtengd-uogan treatment in primary billiary cirrhosis are better than the single drug tre-atment, especially the long course of treatment effect is more significant, it is w-orthy of clinical application.
