| Purpose:To investigate the factors which influence the early elevated intraocularpressure (IOP) after vitrectomy for treating proliferative diabetic retinopathy.Methods:The clinical data of243patients (278eyes) who underwent vitrectomy fromOctober2011to September2014was retrospectively analyzed. We aimed to analyzehow previous intraocular surgery, PDR course duration, stage of PDR, the techniquesof operation, operative time, intraocular tamponade, intraoperative laserphotocoagulation, re-hemorrhage, lens status, inflammatory reaction in anteriorchamber influence the early elevated intraocular pressure.Results:1. The high IOP occurred in141eyes (50.72%) within one week.2. Gender andage had no significant effect on the early elevated intraocular pressure (P=0.275;P=0.473).3. The patients who had intraocular surgery before were more likely to haveIOP elevation than the ones without intraocular surgery (P=0.040).4. There was nocorrelation between PDR course duration and the elevated intraocular pressure(P=0.132).5. The rate of high IOP of PDR VI group was higher than that of PDR IVgroup (P=0.004) and PDR V group (P=0.001), however there was no significantdifference between PDR IV group and PDR V group (P=0.570).6. The techniques ofoperation influenced the IOP elevation significantly, the highest incidence of the highIOP was in20-Gauge vitrectomy and the lowest was in23-Gauge vitrectomy.7. Longoperative time made the occurrence of the IOP elevation more frequent.8. The rate ofhigh IOP of silicon oil group and C3F8group was higher than that of irrigationsolution group (P<0.001; P=0.022), there was no statistically significant difference between the former two groups (P=0.108).9. The rate of IOP elevation betweenpatients without photocoagulation and with partial photocoagulation showed nodifference (P=0.576), those with panretinal photocoagulation achieved higher ratethan the above two groups (P=0.002; P=0.007).10. High IOP was more inclined tooccur in postoperative re-hemorrhage group than in the group without re-hemorrhage(P=0.024).11. The incidence of high IOP in aphakic group was higher than that inautologous lens or pseudophakic group (P=0.001; P=0.001).12. Patients withinflammatory reaction in anterior chamber were prone to the postoperative IOPelevation (P=0.015).13. Logistics regression analysis: the techniques of operation,operative time, intraocular tamponade, re-hemorrhage, inflammatory reaction inanterior chamber are the risk factors of IOP elevation after PPV.Conclusion:1. The early elevated intraocular pressure after vitrectomy is one of the commonpostoperative complications which is caused by a variety of reasons.2. The techniques of operation, operative time, intraocular tamponade,re-hemorrhage, inflammatory reaction in anterior chamber were the risk factors ofintraocular pressure elevation after PPV in patients admitted to our hospital fromOctober2011to September2014. |
