Comparison Of Curative Efect And Complication Of Huge Renal Stone Treatment Between PCNL And MPCNL

Objective: To summarize treatment effect and experience for treating huge stone(stoneburden>6cm) with standard percutaneous nephrolithotomy and microinvasive percutaneousnephrolithotomy.Method: A lot of101patients who suffered from huge calculi were treated with PCNL in Yi Ji Shan HOSPITAL,in which26patients were treated with MPCNL and75patients were treated with standerd PCNL during2001.1to2013.1.Result：PCNL group:Fever rate was24%;11cases(14.7%) need blood transfusion;2cases(2.7%) infected septicemia;4cases（5.3%）had extravasation urine. the total stonefree rate was89.3%，time of operation：106±26.6min。MPCNL group: fever rate was23.1%(6cases);extravasation urine rate was7.7%(2cases); the rate of septicemia was3.8%(1cases);11.5%(3cases) need blood transfusion;The total stonefree rate was69.2%;time of operation：171±30.3min.Conclusions：1、There is no obvious difference in the treatment of huge stones in the surgical indications, feasibility and security between PCNL and MPCNL.2、Standerd PCNL has affirmative effects，and is better in curative effect comparedwith mpcnl for huge kidney stone.3、There is no significant difference in complications bitween standard channels andmicrochannel.4、PCNL has such advantages as little hemorrhage and small incision. However, when it comes to practical application, various factors should be taken into considerationin the choice of an appropriate surgical method,including the characteristics of the patient’s calculi, the economic level of the patients, and the proficiency of the operating surgeon.
