Objective:Through the analysis of breast invasive ductal carcinoma after operation in patients with pathologic characteristics and clinical data, to investigate the distance of infiltration of breast cancer with tumor size, hormone receptor status, Her-2expression, Ki-67expression correlated, safe resection scope to study breast conserving surgery.Methods:54Breast cancer specimens from patients underwent surgical treatments between2010March and2014March were collected, All the54patients are female and aged17to70years, the median age is45.5. Breast ultrasound examination, breast plain and dynamic enhanced MRI, and mammography were performed before operation, pathological diagnosis of malignant breast tumor were obtained by ultrasound guidedcore needle biopsy. All the54patients underwent radical resection of breast conserving surgery or modified operations. The postoperative specimens were prepared into pathological slices, after HE staining, pathological changes were observed and diagnosed by pathologists. Under the microscope to obtain precise direct tumor invasion to surrounding tissue distance, and converted into the actual mirror corresponding to the distance.This experiment using immunohistochemical Her-2SABC method to detect tissue in paraffin sections, with positive paraffin known pieces as positive control, with PBS generation anti as negative control, with HE staining method to determine the properties of paraffin block in the organization, with adjacent normal breast tissue as control.Results:1. In11cases the tumor size less than or equal to1cm (11/54,20.3%),27cases of1-2cm (27/54,50%),16cases of2-3cm (16/54,29.7%).2. Hormone receptor expression were positive in29cases (29/54,53.7%), hormone receptor negative in25cases (25/54,46.3%).3. The expression of Her-2of CerbB-2(-)15cases (15/54,27.8%), CerbB-2(+) in13cases (13/54,24.1%), CerbB-2(++)10cases (10/54,18.5%), CerbB-2(+++)16cases (16/54,29.6%).4. The expression of Ki-67in36cases of negative (36/54,66.7%), the expression of Ki-67positive patients (18/54,33.3%)。5. Tumor size and tumor infiltrating distance correlation (r=0.766, p<0.01), hormone receptor status and tumor infiltrating distance no correlation (r=0.238, P>0.05), Her-2expression and tumor infiltrating distance no correlation (r=0.108, P>0.05), Ki-67expression and tumor infiltrating distance correlation (r=0.578, p<0.01). Conclusion:Infiltrating ductal carcinoma of breast conserving surgery combined with tumor size, lymph node status and so on to decide whether to conserving and conserving required safety range resection.
