BackgroundWallenberg syndrome is also called the Lateral medullary syndrome, which is one of the most common type of brain stem infarction. Because of the large PICA vascular anatomy variation range of sizes, so the syndrome of the patient’s clinical symptom severity difference is bigger, most of the symptom is light, good prognosis; A few symptoms, the prognosis is poor, often die of central respiratory failure. Ever think Wallenberg syndrome is the result of PICA occlusion, with the continuous development of imaging technology in recent years, people of the anatomy, etiology and pathogenesis of Wallenberg syndrome, clinical manifestations, such as with many new understanding, has realized that PICA is not the only cause of the syndrome. In order to study the rule of the syndrome, the improvement treatment measures, improve the level of diagnosis and treatment, thus collected January to November at2013hospitalized in our hospital clinical datas of16patients with Wallenberg syndrome, summarized and analyzed.ObjectiveTo explore vascular anatomy, etiology and risk factors, clinical manifestations, imaging features, diagnosis and treatment, complications as well as prognosis of Wallenberg syndrome, and improve the recognition and diagnosis of this syndrome.MethodsBetween January2013and November2013, clinical data of16inpatients confirmed from MRI with Wallenberg syndrome, were collected from the Department of Neurology, the2nd Affiliated Hospital of Zhejiang University School of Medicine. Reviewing the recent literatures at home and abroad, this syndrome were analyzed retrospectively.ResultsOf16cases,10cases were male, female6cases, age37~85, mean age55.5±9.5years old; Major etiology were large artery atherosclerosis and heart source disease, while The main risk factors were hypertension, hyperglycemia, hyperlipidemia, coronary heart disease, atrial fibrillation and so on. Clinical manifestations are mainly dizziness, nausea, vomiting, bulbar paralysis, contralateral sensory disturbance, Homer indications, ipsilateral cerebellar ataxia five main symptoms, and hearing loss, headaches, eye movement disorder and other rare performance; Imaging of MRI can provide reliable basis for clinical diagnosis. The treatment is the same principle with ischemic cerebrovascular disease; Most prognosis of it is good.ConclusionsBecause of the large PICA vascular anatomy variation Wallenberg syndrome clinical manifestation is complicated, we in addition to master the five classic symptoms of common, should also be familiar with its unusual symptoms. Early diagnosis and early treatment, active control risk factors and prevention and treatment of complications are key determinants of prognosis of Wallenberg syndrome.
