Objective To investigate the related factors of progressive ischemic stroke,especially, the relationship between H-type hypertension and progressive ischemic stroke,thus to provide theoretical basis to predict, diagnosis and treat it.Methods This study collected200cases of hospitalized patients with acute cerebralinfarction in Lianyungang Hospital of Traditional Chinese Medicine from July2013toApril2014. The patients were investigated by Scandinavian Stroke Scale to evaluatewhether the ischemic stroke is progressive. According to plasma Homocysteine (Hcy) andhistory of primary hypertension to judge H-type hypertension. Collecting the patient’smedical history information, laboratory and imaging examination results, to analyses theincidence and the related factors of progressive ischemic stroke, especially, the relationshipbetween H-type hypertension and progressive ischemic stroke.Results The incidence of progressive ischemic stroke was29.0%, among them37.93%occurred in24hours,81.03%occurred in48hours. The single factor analysisfound that related factors of progressive ischemic stroke included: history of primaryhypertension, systolic blood pressure on admission, SSS score on admission, Hcy, H-typehypertension, location of intracranial lesion, intracranial and extracranial artery stenosis.Multiple factor analysis found that H-type hypertension, Hcy, location of intracraniallesion and intracranial artery stenosis were the independent predictors of progressiveischemic stroke, and H-type hypertension is the strongest predictive factor, followed byHcy, location of intracranial lesion and intracranial artery stenosis.Conclusion The incidence of progressive ischemic stroke is high. The progressionalways occurs in early stage of acute cerebral infarction. H-type hypertension, Hcy,location of intracranial lesion and intracranial artery stenosis were independent predictorsof progressive ischemic stroke.
