Objective:To evaluate the value of conventional ultrasound in diagnosis and differential diagnosis of primary gallbladder carcinoma.Methods:318patients with benign and malignant gallbladder disease were examined by conventional ultrasound. The characteristics of conventional ultrasound were analyzed and compared with pathological examination. Analytical focuses include the diagnostic performance of single indicator of gray scale ultrasound signs, the method of differential diagnosis and the causes of misdiagnosis.Results:(1) The single indicator of gray scale ultrasound signs include lesions with irregular shape or gallbladder wall rough, lesions with wide bases or gallbladder wall localized thickening, gallbladder wall discontinuity, Single lesion, the diameter of lesion over10mm or the thickness of the gallbladder wall over3mm, and accompanied with cholecystolithiasis. It was significant different between benign and malignant gallbladder diseases of gray scale ultrasound characteristics (P<0.05).(2) Compared with pathological examination, the sensitivity, specificity, false positive rate, false negative rate, positive predictive value, and negative predictive value of conventional ultrasound in gallbladder carcinomas diagnosis was73.3%、97.2%、26.7%、2.8%、73.3%、97.2%respectively.Conclusion:Conventional ultrasound has an important clinical value in diagnosis and differential diagnosis of primary gallbladder carcinoma. The diagnostic performance of single indicator of gray scale ultrasound signs has a directive significance in diagnosis of benign and malignant gallbladder disease.
