Objective:In2009, the new medical reform was started, marked the return of public healthcareer. Basic medical and health institutions as providing the basic services ofprimary care and public health services agency, is the public nature of the mostdirect, most people first, the bottom of the unit. Also because of maintaining publicnature, prompted the original benefit structure changed, the reconstructedcompensation, allocation, and protection mechanisms to ensure for the people withsafe, effective, convenient and inexpensive essential health services. Then to studywhether new multi-channel compensation mechanism can meet the needs of primaryhealth care institutions, institutions operating factors exist which hinder thedevelopment and mechanisms which bottlenecks encountered ininstitution-building, and for deepening the comprehensive reform of basic medicaland health institutions in our province, to further improve related policies andmeasures, with realistic and profound significance.Methods:This research through literature review, understanding the cost keeping at homeand abroad, and the research status of compensation mechanism, to get on costoperation of principle and method; through the survey of field interview and massessatisfaction degrees survey, understanding basic medical health institutionsadministration personnel, and health personnel and medical patients on grass-rootsintegrated reform of views and the effect, started qualitative analysis; throughfinishing large practice data, classification accounting institutions run cost andcompensation situation, get grass-roots medical health institutions constructionstatus, on associated data individually classification compared, quantitative research,identify problems find the reasons; theoretical derivation to design policies, putforward suggestions of perfecting compensation mechanism. Results:Current province primary health care institutions financial investmentincreased year by year, to improve infrastructure facilities, the number of beds perthousand population served up to the national standards, medical staff and presidentsfeel the positive changes,but health staffing shortage,lack of professional andtechnical personnel, did not fully meet the standard level of the building;Agencyrevenue underpowered, performance appraisal incentive is not strong, medical staffare not enthusiastic, institutional development was shrinking;Reduced output ofmedical services, institutional services decline, public health service labor costs aretoo high, is not conducive to public health and health services;Problems from thepoint of view, should focus on resolving the key issue of financial investment,personnel development, financial management and performance distributionsystem.Conclusion:In this study for health administrative departments at all levels to deepen reform,and improve the compensation mechanisms, provides a systematic andcomprehensive research, provides basic medical and health institutions to carry outcost accounting with a reference. Studies suggest that in our province to establishnormal multi-channel compensation mechanism can basically meet the institutionsrunning, but existing financial management system does not meet the needs ofhealthy development of the basic medical and health institutions, should take theinitiative to conduct research and to adjust the structure of fiscal expenditure for thecore to develop related policies and measures, establishment of compensationmechanism of the reform process.
