Clinical Curative Effect Analysis Of Hemangioma In360Patients With Cutaneous Vascular Anomalies

Objective: The aim of this study is to evaluate the efficacy and complications ofpropranolol and laser in treating cutaneous vascular anomalies.Materials and methods: A total number of360patients with cutaneous vascularanomalies were collected from2012January to2014January in department ofDermatology of China-Japan Union hospital. Of the360cases,150were infantilehemangioma (IH)(116were strawberry hemangioma,19were mixed typehemangioma,15were cavernous hemangioma),10were pyogenic granuloma,85were nevus flammeus,115were spider nevus. there were150male cases and210female cases were involved in this study, with sex ratio1:1.4（male: female）. the ageof onset was from at birth to24years; the median age was3.9years old; the age ofdiagnosis was from1months to50years old, the median age was8.8years old. Ofthe total418various types of abnormal vascular lesions, strawberry hemangioma ismost common (154lesions), followed by the spider nevus(121lesions),99werenevus flammeus,19were mixed hemangioma,15were cavernous, and10werepyogenic granuloma. Of the total418lesions,283were in the head and face,64werein the neck and trunk,71were in the limbs and perineum;335lesions were small,35were medium-sized,33lesions were large,15lesions were giant.322cases weresingle lesion,38cases were with multiple lesions. Of150cases with infantilehemangioma,119cases (79.3%) were in the proliferative phase,31cases (20.7%) inthe involuting and involuted phase. Of150cases with infantile hemangioma, lowweight at birth was seen in28cases, accounting for18.7%.5cases (1.4%) patientshad family history. Different treatment methods were applied according to the diseasetypes, and the effectiveness of the treatment was assessed. Cavernous hemangiomaand mixed hemangioma were treated with oral propranolol (2mg/kg/d) combined withadjustable pulsewidth1064nm Nd:YAG laser treatment；strawberry hemangioma,nevus flammeus, spider nevus, and pyogenic granuloma were treated with595nm pulsed dye laser.Results: The cure efficiency of spider nevus was the highest,which was100.0%; following that of the strawberry hemangioma,82.8%; then, the pyogenicgranuloma,80.0%; then the cavernous hemangioma,73.4%.Conclusion:1. Take propranolol orally in combination with adjustable pulsewidth1064nmNd:YAG laser is effective and safe in the treatment of mixed type hemangioma andcavernous hemangioma.2. Use595nm pulsed dye laser in the treatment of strawberry hemangioma,pyogenic granuloma, nevus flammeus and spider nevus can reduce the incidence ofscar. And the cure efficiency can reach to80%-100%except for nevus flammeus.
