A Retrospective Analysis Of Surgical Treatment Of4703Cases With Breast Cancer

Purpose:To review the evolution of the surgical treatment for breast cancerover the past thirty-four years in the First Hospital of Jilin University (theformer Bethune University of Medical Sciences).Materials and Methods:This research retrospectively analyzed4703patients diagnosed withprimary breast cancer and surgically treated in the First Hospital fromJanuary1980to March2014. The youngest is17years old, and the oldestis84years old. The average age is46.7years old.4670(99.30%) patientsare female, and33(0.70%) are male.50.71%of tumors occurred in theleft breast,47.99%in the right,1.30%in the bilateral breasts. Patientswere clinically and pathologically divided into stages by UICC,2003.There were91（1.93%）cases in stage0,1263（26.86%）in stage Ⅰ,2533（53.86%）cases in stage II,749（15.93%）cases in stage III and67（1.42%）cases in stage Ⅳ. Lobular carcinoma in situ:36case（s0.76%），Ductal carcinoma in situ:84cases（1.79%），Infiltrative duct carcinoma:4080case（s86.75%），Infiltrative lobular carcinoma:207case（s4.41%），Mucinous adenocarcinoma:80cases（1.70%），Paget’s disease:30cases（0.63%），Others:186case（s3.96%）.Modified radical mastectomy3402（72.34%）,Total mastectomy+SLNB419（8.91%）,Lumpectomy+ALND382（8.12%）,Lumpectomy+SLNB151（3.21%）.Some typicalmethods,such as Chi-Square (χ2) test or Fisher’s Exact Probability method, were applied to do statistical test. All statistical computationswere performed by use of SAS software (Ver.19.0).Results:The number of patients has been dramatically increased in the past10years. The peak age was in41-50years old，the second was51-60years old. The most common pTNM stage was Stage II. The rate of stage0and stage I has been increased since2000. The most commonhistological type was Infiltrative duct carcinoma, the second wasinfiltrative lobular carcinoma. The most common intrinsic subtype ofbreast cancer was Luminal A, the second was Luminal B. The rate ofSLNB and lumpectomy has been increased in the past10years. Therecently most common operation was modified radical mastectomy.Conclusion:1. The number of patients has been dramatically increased in the past10years, which may inflect the incidence of breast cancer isincreasing in Jilin.2. The peak age incidence of breast cancer was41-50years old, thesecond was51-60years old.3. The rate of stage0and stageⅠhas been increased recently.4. The progression of operation procedures performed on patients withbreast cancer was radical mastectomy–modified radicalmastectomy–breast conserving surgery–SLNB.5. The most common intrinsic subtype of breast cancer was Luminal A,the second was Luminal B.
