Objective:To determine the effect of expectant management in early onset severe preeclampsia on maternal and perinatal outcomes.Methods:137patients’data with severe preeclampsia who delivered in Nanjing Drum Tower Hospital form January2006to January2012were analyzed and the mothers and their infants were followed up in postpartum6months. The patients were divided in to3groups according to their time of expectant management:Group I: pregnancy termination in2days after hospitalization; Group Ⅱ:pregnancy termination in3-7days; Group III:pregnancy termination in more than7days after hospitalization.Results:The maternal clinical characteristics of age, blood pressure and urine protein amount were the same and of no statistical significant difference(P>0.05). The incidence of maternal and perinatal complications was also of no significant difference among the three groups (P>0.05). The perinatal morbidity and mortality were60.5%,46.2%,30.9%and53.5%,41.0%,21.8%respectively and the difference was also of no statistical significant (P<0.05). The mother chronic hypertension incidence was19.7%and the infant abnormal intelligence development rate was18.6%, The incidence difference among the three groups was of no statistical significant (P>0.05).Conclusions:Expectant management is possible in some women with early onset severe preeclampsia and their perinatal outcomes are improved obviously, but this disease increase the risk of chronic hypertension and abnormal intelligence development of mother and infant.
