Hypnosis Effected On The Population During Low Epidural Anesthesia With High Hypnotic Susceptibility

Objective:Discuss high hypnotic susceptibility population during low epidural anesthesia affected by hypnosis-suggesting.Methods:Chose from low epidural anesthesia patients,use SGHSS:C,score≧7.SCL-90,each item score<2,Left and right handedness scale,right handedness.30patients.Operation time during1～2hours, ASA Ⅰ～Ⅱ, age:18-65,body mass index:18～30kg/m2.Excluding long-term use of central inhibition of drugs,Endocrine disease history,Mental retardation, hearing impairment, mental disorder,were randomly divided into three groups:hypnosis group(H), vein group(V), control group(C).Each group10patients. Both group gave Conventional dosage atropine0.5mg, phenobarbital0.1pre-operation30minutes. Choose L2-L3to T12-L1interval for puncture, epidural catheter fixed ends,Gave3ml of2%lidocaine test after no adverse reaction,1%ropivacaine7～10ml, measurement anesthesia plane reach T8-L2meet the needs of the operation. Oxygen masks, when operating time more than1hour,give1%ropivacaine3～5ml again,if in pain intraoperative, excludable. Hypnosis group of ten minutes before the surgery to give professional music hypnosis and hints, vein group10minutes before the start of operation intravenous injection of propofol injection pump2～4mg/kg.h, control group routine epidural anesthesia only. Separate determination:before anesthesia To after cut skin5minutes T1intraoperative30minutes T2.the end of operation T3. Heart rate and blood sugar. BIS continuous monitoring of anesthetic depth during operation. VAS scale and comfort scale measure the degree of postoperative patients of pain and comfort ratings. Results:Group C and group V heart rate on the decline and in the normal range.compare with To statistical significance (P<0.05). While the control group in the heart rate rising and volatile, compare with group C and group V statistical significance (P<0.05). Compare with To both group blood sugar rise but group C and group V under group C statistical significance (P<0.05) and in the normal range. Group C blood sugar beyond the normal range, the stress hyperglycemia. During the operation,group V BIS90～100,group C BIS65～85,in a state of calm.group V BIS45-65, narcotism.when group V and group C had the same BIS,group V need intravenous injection of propofol1～2.5mg/kg.h.VAS scale3groups no statistical significance (P>0.05) intraoperation.Comfort ratings, vein group and hypnosis group were higher than control group significantly alleviate psychological pressure (P<0.05)Conclusion:Hypnosis applied in high hypnotic susceptibility in patients with epidural anesthesia, reduced the patient’s stress response. Patients with stable heart rate, reduce myocardial oxygen consumption, to prevent intraoperative irritable hyperglycemia. Monitoring by BIS, under the intervention of hypnosis hypnosis high susceptible patients during anesthesia, can reach a state of calm. For patients with postoperative VAS score, found that the three groups of patients with postoperative pain no difference (P>0.05), suggesting that the analgesic effect of epidural anesthesia perfect mask the hypnotic effect on pain. But can be seen from the comfort rating, comfort of hypnosis group was obviously better than control group (P<0.05), hypnosis psychological intervention is effective on the patients, to eliminate patients for surgery, anesthesia of fear, anxiety, nervousness. In operation under the action of this kind of intense stimulation factors, patients can be achieved through hypnotic sedative state, reduce the adverse effects of stress, improve the patient’s comfort, stable circulation system, and reduce the economic burden of the patients.
