Objective:To explore the incidence rate and analyse the related risk factors of braininjury in premature infants and provide the basis for the brain injury in preterm infants ofperinatal prevention and early intervention, and improve the quality of life in preterminfants.Method:From March,2012to February,2013,166preterm infants selected as theresearch object from department of Neonatology of Soochow University AffiliatedChidren’s Hospital.According to the results of cranial ultrasound, brain CT and brainMRI,one side,the subjects were divided into non PVH-IVH group (121cases),45cases ofPVH-IVH group:the gradeⅠ PVH-IVH (20cases), gradeⅡPVH-IVH (13cases), grade ⅢPVH-IVH (7cases), grade Ⅳ PVH-IVH (5cases);On the other side,the subjects weredivided into non PVL group (114cases),52cases of PVL group:the gradeⅠPVL (26cases), gradeⅡPVL (15cases), grade Ⅲ PVL (8cases), grade Ⅳ PVL (3cases).Comparedthe related risk factors of each groups to find the differences.A statistically significantfactor analysis of single factor, further multivariate Logistic regression analysis,to explorethe high related risk factors of brain injury in premature infants.Results:1. From the166preterm infants selected,45(27.11%)cases are PVH-IVH,52(31.33)cases are PVL.the gradeⅠPVH-IVH20cases(44.44%), gradeⅡPVH-IVH13cases(28.89), gradeⅢPVH-IVH7cases(15.56), gradeⅣPVH-IVH5cases(11.11);thegradeⅠPVL26cases(52%), gradeⅡPVL15cases(28.85%), grade Ⅲ PVL8cases(15.38%), grade Ⅳ PVL3cases(5.77%).2.the relationship of the gestational age,birthweight with the incidence of PVH-IVH,PVL show:Preterm infants with a gestational ageof the smaller, more low birth weight, PVH-IVH and PVL formation rate is high.the degreeof perinatal asphyxia have correlation with the incidence of PVH-IVH,andPVL(p<0.05).The degree of asphyxia was significant,the incidence of PVH-IVH and PVL is higher.3.Multivariate logistic regression analysis revealed:Gestational age as risk factorshave positively correlation with preterm infants with brain injury (p<0.05).Conclusion:1.The incidence of PVH-IVH and PVL is relatively high in prematureinfants.Preterm infants with a gestational age of the smaller, more low birth weight,moreserious of asphyxia,PVH-IVH and PVL formation rate is high.2.The gestational age, birthweight, perinatal asphyxia degree, mechanical ventilation, oxygen, mode of delivery,single/multiple births and so on the many kinds of factors have influence with brain injuryin premature infants,in order to prevent the occurrence of BIPI,comprehensive measuresshould be taken.
