Surgical Approach,Significance And Literature Review Of Local Regional Lymphadenectomy In Advanced Stage Gallbladder Carcinoma

Objective:There were many controversies on the extent of lymphadenectomy in advanced stage gallbladder carcinoma. We aimed to summarized the experience of local regional lymph node dissection in advanced stage gallbladder carcinoma and explore the experience of this surgical procedure.Methods:From December2008to July2013, total of twenty three patients with gallbladder carcinoma were retrospectively analysed, including general data, imageology, perioperative treament, postoperative pathology and follow-up results.Results Of all23patients,7were male and16were female.The median age was64year-old with age ranged from45to85year-old.18patients had gallbladder stone or chronic cholecystitis. Totally21cases accepted B ultrasound and18cases accepted CT scan, with the accuracy of was52.38%(11/21) and72.22%(13/18), respectively. Conservative treatment for8cases (34.78%) and surgical therapy for other cases (61.11%).11cases underwent radical resection, palliative treatment in3and simple cholecystectomy in1.11case were diagnosed adenocarcinoma,1was small cell neuroendocrine carcinoma,1was papillocarcinoma,1was intraepithelial neoplasia with focal cancerous and1was moderate or severe dysplasia with focal cancerous.21cases have been followed-up and the rate of91.3%(21/23). The over all survival time ranged from1to38months with mean time was19.8±3.87months. The mean survival time was16.14±6.37months (1-38months) with supportive treament, and it was21.68±4.36months with surgical resection. There was no statistical difference between the two groups.Conclusion There was no increasing rate of operational complication for localregional lymph node dissection. And this approach was significant for the quality of lymph node dissection in advanced stage gallbladder carcinoma.
