Backgroud:Acinetobacter baumannii have acquired resistance and strong ability to communicate cloning, multi-drug resistant, extensively drug-resistant, all-resistant Acinetobacter baumannii was the world’s epidemic has become one of the most important pathogen of nosocomial infections. Since the resistance of Acinetobacter baumannii there are regional differences and hospitals, clinicians should be aware of the local hospitals, especially where drug monitoring results, the hospital where the study of its past six months, Acinetobacter baumannii cultivate the growth of patient data a retrospective analysis to explore Acinetobacter baumannii lower respiratory tract infection by clinical distribution, drug resistance and antibiotic therapy for clinically effective prevention of Acinetobacter baumannii infections and provide the basis for rational use of antibiotics.Objective:To analyze the clicinal distribution and resistance of Acinetobacter baumannii, and outcome of pneumonia cases caused by A. baumannii.Methods:During January2013to June2013, a total of420strains of A. baumannii were isolated in the First Affiliated Hospital, College of Medicine, Zhejiang University. Clicinal distribution and resistance of A. baumannii was analyzed. Clinical feature and outcome of A. baumannii pneumonia cases were also analyzed.Results:Among the420A. baumannii stranis,193were isolated from ICU(45.96%),152were from the department of surgery (36.19%). Total of66.9%strains were isolated from sputum and8.81%from blood cultures. In79cases (18.8%), A. baumannii was isolated form more than1site. Among62cases, A. baumannii were isolated from sputum firstly and then from other sites. In this study, the resistant rate of A. baumannii to amikacin, tigecycline, levofloxacin, cefoperazon/sulbactam, ampicillin/sulbactam, imipenem, meropenem were5.2%,15.9%,35.6%,64.2%,71.1%,79.1%,88.9%, respectively. Total of72A. baumannii pneumonia casese were collected. The crude death rate was19.4%. The C-react protein (CRP) level of survival case and death cases were69.5mg/L and137.6mg/L(P<0.05). APACHE II score of survival cases and death cases were16.4and19.7(P<0.05). Conclusion:A. baumannii is mostly isolated from patients admitted in ICU or underwent surgery. In18.8%cases, A. baumannii was isolated form more than1site. Because of high resistance, it is difficult to treat the infection of A. baumannii. CRP and APACHE Ⅱ scroe are the prediction factors of outcome in pneumonia cases.
