Objective:To determine the differences in practice between laparoscopic myomectomy(LM) and abdominal myomectomy(AM),also,To discuss the safety.feasibility and indications of laparoscopic myomectomy.Methods:We analyzed data for231LMs and66Ams performed from Jan2010to Jan2011in the women" s hospital of Zhejiang University School of Medicine. The patients must be younger than40years old. Compared the pre-,per-and post-operative instances.Results:The operative time, the ratio of through uterine cavity were similar in the two groups. Compared with AM, LM had less bleeding, lower ratio of multiple myoma [32.9%(76cases) vs AM97.0%(64cases)], less diameter of the biggest myoma[(6.0±1.6)cm vs AM(7.2±3.2)cm],all the differences had statistic significance. The ratio of retention of urine was similar in the two groups.Compared with AM,less advent of fever post operation [1.73%(4cases) vs AM12.12%(8cases)], less ratio to use analgetic post operation[7.79%(18cases) vs AM34.85%(23case)], shorter hospital time(7.4±1.6)days vs AM(8.2±1.9)days], and all the differences had statistic significance.The ratio of uterine myoma recurrence or residual,the ratio of patients without improvement post operation/the ratio of reoperation were similar in the two groups.The pregnancy rate for LMs was14.3%(3leases) vs AMs22.7%(15cases), no scar rupture occurred in these pregnancies.Conclusion:Laparoscopic myomectomy has the advantage of a minimally invasive operation, and it is safe and effective.It is deserved to be widely used,and its indications will be expanded,but it can not completely take place of the abdominal myomectomy... |
