Abstract:Objective:By measuring dental implants time after sites save surgery’s influence on the stability of the implant, and observe newborn-induced bone to bone during the biomechanical properties and clinical characteristics of the different stages.Forecast the initial stability of the implant surgery in different times after GBR surgery, thus observe the clinical features in different stages of newborn-induced bone into bone and refer to to the timing of implanting the Implantation after clinical GBR surgery. Method:The sample selected from January2010to December2012,58patients to the Dentistry of the Chengdu Military Region Authorities Hospital (all male, aged40-60years, mean:48.64+8.641)69posterior mandible (mandibular6,7),divided into A、B and C groups. All subjects to be surgical regional alveolar distance of the top of the mandibular canal is greater than or equal to10mm, the width is greater than or equal to7.1mm the adjacent teeth probing no attachment loss. Inclusion criteria:patients with mandibular teeth diagnosed with tooth fractures or deep caries and can not be reserved, unplug by GBR surgery, and then implement of the implant surgery and surgery before the X ray shows the length of the alveolar crest to the apicalat equal to 8.5mm.1. Disease patients can not undergo surgery;2. Systemic diseases such as diabetes, osteoporosis, etc.;3. Psychological or psychiatric disease which can affect patients’daily oral maintenance and can not undergo surgery because of psychological factors;4.The patients which have severe gingivitis,severe periodontal disease and not be controlled Full mouth near and far in the proximal,and Implant adjacent teeth with apical lesions or activity with periodontal disease;5. patients with bruxism, clenching and other symptoms or alcohol abuse, long-term heavy smoking and other bad habits.The sample size of69samples randomly alternating divided into chronological newly diagnosed treatment GBR after6months (group Al),9months (group B), lyear (C group) implant surgery three groups of23cases. The object of study of this experiment are Select Germany Bego diameter4.1mm, length8.5mm implant during implant. Prepare the osteotomy standard method respectively. In the implant surgery,when Implant is fully seated, to manual measurements implant torque through a ratchet wrench/torque control wrench.To recorded patients’ abnormal sensation when the torque value is30N.cm in the central screw is screwed in the two repair surgery which Screwed into the central screw in the two repair surgery. All operations were performed by the same physician. Results:1.We found A, B, C group implant surgery:Mucoperiosteal flap flips in the original biofilm coverage area have the new bone, new bone surface uneven, still visible small amounts of unmetabolized granular bone substitute materials, with the newborn induce bone to bone extension of time, the bone substitute material particlesgradually reduced or even disappear, but no new bone surface cortical bone formation.2.A, B, C group operation area implants the nest have fresh blood overflow from the new bone,drill2.5mm crack fissure bur resistance trend to increase with osteogenesis time to determine planting depth, andunder paragraph resistance was significantly greater than the previous paragraph.3. When we observed in the periapical radiographs of the oral X line, A, B, C group of new bone and the boundary of the original ridge there is always locus save area was significantly higher than that of the surrounding natural alveolar bone, bone time with new boneboundary blurring, the sites preservation district bone mineral density decreased.4. Implant torque A<B<C, a statistically significant difference (P<0.05), new bone implant torque before implantation into bone time was positively correlated.5. A, B, C group row the two repair preoperative patients had no numbness, pain and discomfort, around the implant X-ray transmission region, flap surgery clinical loosening of the implant had no abnormalities found, the surrounding alveolar boneedge no significant absorption. Surgical repair of the central screw afterburner to30N.cm patients showed no implant loosening, pain and other abnormal sensations.6. A, B, C group function load March, the edge of the alveolar bone absorption of new bone after bone extension of time saved with the early sites, gradually decreasing trend. Conclusions:1. The new induced bone of Sites saved intraoperative was concentric growth of new bone formation,and is accompanied by the growth of blood vessels. The sites save newborn after bone into bone extension of time, the bone substitute materials gradually metabolized, newborn induce bone instead of new bone in the process of formation and remodeling gradually blend with the surrounding natural alveolar bone.3.Sites year after save, to induce bone into bone extension of time, with the newborn, implant the torque gradually increasing trend, confirmed the induction of new bone over time to mature.4.Whether the reign of points saved the postoperative June, September or a year of implantation, no significant difference in the stability of the implant prosthetics.5. Implants the function load after March, neck alveolar bone resorption newborn with implantation induced bone to bone time. The less the newborn before implantation induced osteoblasts time longer, the repair of the postoperative functional load-month implant marginal alveolar bone uptake. The degree of maturity of the newborn-induced bone for implant function to load the early crestal alveolar bone resorption have a certain impact.
