Objective:The colorectal carcinoma is a common tumor and ranks second in incidence in the western developed countries. The incidence rate of the colorectal carcinoma increases. Due to the particularity of the colorectal carcinoma, the treatment effect of the carcinoma is not good enough. The recurrence rate is high. In order to reduce the recurrence rate and improve the survival, neoadjuvant therapy were developed, and it improved treatment of the rectal carcinoma. The neoadjuvant chemoradiation therapy can result in reduction in the local recurrence rate and may increase the ability of the surgeon to preserve continence by down-staging the cancer.The preoperative staging of rectal cancer is the basic of treatment plan that we should make. And the MRI which is the main way to confirm the diagnosis of rectal cancer is particularly important. The aim of this study is to explore and analyze the value of magnetic resonance in the preoperative staging of rectal carcinoma. Methods:The clinical data of one hundred and twenty-eight patients with rectal carcinoma received the magnetic resonance scanning and imaging analysis was investigated. The preoperative MRI staging was compared with the postoperative pathological TMN staging. Also the relationship between Sex, age, body mass index, the distance from the tumor to the anal verge, the proportion of the circumference of bowel involved by tumor and the accuracy of preoperative MRI staging were analyzed.Results:1. Among the128cases, the accuracy rate of MRI in the judgment of T stage was57.03%. The accuracy rates of T1, T2, T3, T4stages were40%,44%,60%,61.54%, respectively (kappa=0.410, P=0.000).With the T stage increased, the accuracy rate increased.2. The accuracy rates in theTI+T2group and T3+T4group were50%and90.82%, respectively (K=0.443,P=0.000).3. The accuracy rate of MRI for the judgment of N stage was72.66%.The accuracy rates of NO stage and N阳stage were59.21%and92.31%, respectively (K=0.718,P=0.000).4. Sex, age, body mass index,the distance from the tumor to the anal verge and the proportion of the circumference of bowel involved by tumor have no relation with the accuracy of preoperative MRI staging of rectal carcinomaConclusion:1. The magnetic resonance Imaging can provide fairly accurate assessment of the extent of invasion of the bowel wall in T3and T4rectal cancer. MRI plays a great role in the preoperative staging and treatment of rectal carcinoma. 2. The sex, age, body mass index, the distance from the tumor to the anal verge and the proportion of the circumference of bowel involved by tumor have no relation with the accuracy of preoperative MRI staging of rectal carcinoma.
