objective: To explore the curative effect of using endoscopicsequential treatment treat esophageal cancer patients in progressive stageand the optimal scheme of sequential treatment. Methods:240patientswere divided into experimental group and control group, among them,there are126cases in experimental group also namely sequentialtreatment group, and there are114cases in control group also namelysimple treatment group. Among them, experimental group was dividedinto group A: local chemotherapy drug injection+bare-metal stentinggroup, group B: local chemotherapy drug injection+iodine-125particlestenting group, group C: radiofrequency treatment+bare-metal stentinggroup, group; Control group was divided into group D: simple localchemotherapy drug injection group, the group E: pure radiofrequencytreatment group, group F: simple bare-metal stenting group. Experimentalgroup according to the level of difficulty swallowing, were divided intotwo stages to treat.At the fist stage, the group A and group B used localchemotherapy drug injection for treatment respectively, and the group Cused radio frequency (rf) for treatment. After a period of treatment,evaluated the swallowing trapped of the patients according to the Stoolerclassification, and different levels went into the second stage of treatmentin different time. If his difficulty swallowing level was in I–II, he cango into the second stage of treatment when his swallowing levelprogress to the III-IV, that is to say, the group A and group C canimplanting ordinary mental stent, and the group B can implanting iodine-125particle stent, and implant another ordinary mental stent when therestenosis occurs. If his difficulty swallowing level was in III-IV, he can group A and group C can implanting ordinary mental stent, and the groupB can implanting iodine-125particle stent, and implant anotherordinary mental stent when the restenosis occurs. The control groupsimply used local chemotherapy drug injection, radio frequency (rf) andstenting for treatment. The group D and group E, after a period oftreatment, evaluated the difficulty swallowing level of the patientsaccording to the Stooler classification. If his difficult swallowing levelwas in I–II, he can go into the next period of treatment when hisswallowing level progress to the III-IV. If his difficulty swallowinglevel was in III-IV, he can go into the next period of treatmentdirectly. The group F implants ordinary mental stent simply, andimplant another ordinary mental stent when the restenosis occured.Compared the survival rate and the quality of life of experimental group in3months,6months,1year,1.5year,2years with which of the controlgroup, and compared the adverse reaction happening and of theexperimental group with which of the control group, and analyzed theresults. Results: The survival rates of the experimental group in6months,1year,1.5year,2year were higher than the control group, among them,the survival rate of group A and group B in3months,6months,1year,1.5year,2year, were the same, while he survival rate of group A andgroup B were higher than that of group C, in6months,1year,1.5year,2year. In terms of quality of life, patients’ overall health, physical function,social function, emotional function mean score after treatment1year,1.5year,2years were all higher than that of the control group, and the meanscore of group A and group B were higher than that of group C and groupD, while, the mean score of group B was higher than group A. Incidence of adverse reactions was no statistical difference in both experimentalgroup and control group, and the esophageal tracheal fistula and newtransfer rates of group A and group B were significantly lower than that ofthe rest of the group, while that of group B were lower than group A.Conclusion: Using endoscopic sequential treatment to treat esophagealcancer patients in progressive stage, the curative effect was distinct. Inthese methods the local chemotherapy drug injection+iodine-125seedsstent implantation was the most ideal treatment, while local chemotherapydrug injection+ordinary stenting treatment, was also the ideal treatment.
