Pain often develops during orthodontic treatment process. Orthodontic pain discomfort during treatment often influences the patient’s feelings, emotions and daily life. Painful discomfort affects the enthusiasm of patient; some patients intentionally delay the appointment date. This could lead to a worse result of orthodontic treatment. Painful discomfort is now one of factors which influence the patients’ willing to continue the orthodontic treatment or not. Many steps during process of fixed orthodontic treatment could cause painful feeling and discomfort. How to reduce or even eliminate pain discomfort during orthodontic treatment has significant difference for patients to cooperate actively.A passive self-locking bracket is simple and convenient to operate, reduce operation time for the dentist. In this way, the comfort feeling of self-locking bracket has improving utilization rate of self-locking braces. Because of these features, self-locking braces are widely used. Some scholars reported that relative soft force can relieve the pain and discomfort during the treatment fixed device. Self-locking brackets can offer soft force during treatment f patients choose it. The self-locking bracket is more comfortable than that of traditional brackets stays only on the theoretical level. But there are also some results do not support the above view.Objective:Clinical questionnaire investigation was used to compare difference of orthodontic pain caused by self-locking and traditional brackets on the early stage. The orthodontic pain and its change were also explored in this study. Provide theoretical support for clinical orthodontic pain control, and to guide clinical operation.Methods:110patients aged12-18(72of female and38male) who began orthodontic treatment, from stomatological hospital, Lanzhou University were selected during the period from September2012to December2013. Then the patients randomly divided into self-locking brackets group and traditional brackets group.Visual simulation rating scale (visual analogue scale, VAS) was used to evaluate the pain discomfort after2h,6h,18h,42h,72h and168h of treatment respectively. At the same time, a simplified McGill pain questionnaire (SF-MPQ) evaluation was used to access the character of pain. And then statistical analysis was conducted to compare the pain intensity and nature of any differences between the two groups. Questionnaires were retrieved and results were concluded.Results:1.110questionnaires were issued, and92of them are valid. Retrieving rate is83.64%. There’s no statistical difference on painful discomfort between (P>0.05) self-locking and traditional brackets in the early stage in the first week. There’s no statistical difference (P>0.05) between Self-locking and traditional brackets on the nature of pain.2.15.22%of the participants did not heard of orthodontic pain discomfort related information; In early wear of fixed orthodontic correction in the first week,100%of patients with pain discomfort.30.43%of the subjects because of the pain and discomfort to the past in addition to the fixed appliance; Pain discomfort after first appeared wearing fixed appliance after0.5h, appeared in the latest15h, after an average of2.75h; Pain discomfort disappear time first appeared in1d, the latest in8d, starting from2-3d pain gradually reduced to7d, pain discomfort almost disappear. Pain discomfort shortest duration of1d, up to8d, an average of4.5days; At the beginning of the fixed orthodontic correction device wear, in the first week pain discomfort appear similar to the change of the parabolic type.3.Patients experience in the first few hours feel teeth have tight feeling, slight pain, less discomfort, do not affect pronunciation, eating and mood. In patients with2-3d, clearly feel bite, tenderness, discomfort, eating and mood; In6-7d, discomfort, characterized by mild pain and itch, does not affect the life; Ulcer incidence of54.35%, more than occurred in3-4d, location of fangs, molar area in the back; Subjects relieve pain after early wear and correcting device is not the way is:the distraction. Pain discomfort an average time of2.75h, with the passage of time, the. pain discomfort continuously strengthen, at48-72h, peaked pain, discomfort, pain, discomfort after gradually reduce, to the7d basic disappear;4.Patients experience in the first few hours feel teeth researchConclusions:1. There’s no difference on the patient’s pain intensity and the influence of nature between different types of brackets. 2. The occurrence of orthodontic pain discomfort is the inevitable reaction after applying orthodontic force; it has certain regularity.3. Orthodontic painful discomfort at the earliest time since the beginning of treatment after0.5h,and appear the latest after15h of the treatment. The pain gradually disappears in4.5days.4. Orthodontic pain does present a class parabolic curve. The peak is at48-72h, and then gradually declined.5. There’s a certain correlation between the orthodontic pain and the nature of the pain.
