Objective: The aim of the study is to screening for sepsis-related serum microRNA byusing genome-wide microRNA profiling microarray and to validate some of thedifferentially expressed microRNA by quantitative reverse transcription-polymerasechain reaction (RT-qPCR).Materials and Methods: Sepsis patients and non sepsis controls were selected fromrespiratory intensive care unit (RICU), emergency intensive care unit (EICU). Serumsamples were collected from patients on enrollment. Second serum samples werecollected when sepsis patients’ condition improved. Through the comparison of sepsisstage and condition improvement stage by Microarray technology, sepsis-related serummicroRNAs were screened. And then some of which were validated by RT-qPCR in20sepsis patients. Furthermore, we evaluate its diagnostic value by comparing sepsispatients (n=95) with non sepsis patients (n=66).Results:(1) Three sepsis patients with confirmed bacterial infection, who were treatedsuccessfully, were selected for microarray study. A contrastive analysis of serummicroRNA profiling was made, and20differentially expressed microRNAs betweensepsis stage and condition improvement stage were found.(2) Serum hsa-miR-3591-5p、hsa-miR-501-5p and hsa-let-7d-3p were verified by RT-qPCR in20sepsis patients, onlyhsa-let-7d-3p has a statitical difference between sepsis stage and condition improvementstage.(3) We enrolled95cases of sepsis patients and66cases of non sepsis controls.The levels of serum hsa-let-7d-3p in sepsis group were significantly higher than thecontrol group. In a multiple logistic regression model, serum hsa-let-7d-3p was found tobe independent predictor of sepsis. Receiver operating characteristic curve (ROC) analysis showed that the area under ROC curve of serum hsa-let-7d-3p was0.696(95%confidence interval [0.615,0.778]).Conclusions: By contrastive analysis of the serums microRNA profiling, differentiallyexpressed microRNA between sepsis stage and condition improvement stage werescreened. Serum hsa-let-7d-3p were verified to be associated with sepsis by RT-qPCR, ithas the potential to be a new biomarker in early sepsis recognition.
