【Objects】To probe the clinical application and value of dual source CT quantificationvolume imaging to forecast lung cancer patients’postoperative pulmonary functionchanging.【Methods】Between June2012and June2013,233patients who accepted the singleoperation hole VATS lobectomy or holo-lungs pneumonectomy conductedpulmonary function tests before and after3months of the surgery,dual source CTvolume scan was conducted at both inspiration phase and expiration phase beforethe surgery and used workstation to measure the lung volume of the single lunglobe and the whole lung, pixel exponential distribution histogram, and the averagelung density.Then compared the discrepancy and correlation between the volumeaccepted by CT and preoperative, postoperative pulmonary function index aswell as the lung density, compared the pulmonary function change of differentlobectomy before and after the surgery,analyzed every lobes percentage of thewhole lung, all the data were analyzed with SPSS20.0software, the quantitativedata was analyzed in “t” test before and after operation in patients with pulmonaryfunction difference, between many groups of quantitative data are compared withsingle factor analysis of variance,compared each pair-group with LSD method, p <0.01indicates the differences had statistical significance. 【Results】1:For the233patients in the research,the average age was53±15.6year,121male,112female,51patients accepted right superior lobectomy,12right middlelobectomy,39right inferior lobe,41left superior lobectomy,43left inferiorlobe,17the right whole lung and30the left whole lung.215NSCLC patients,14cases were metastasis beyond lung,4cases were lymphoma.2: Compared the CT volume data from patients before the operation and routinepulmonary function index, the highest correlation coefficient inTLC,FVC,FEV1,RV,FEV1/FVC were0.92,0.76,0.70,0.78,-0.51.3: Compared the CT volume data from patients after the operation and routinepulmonary function index, the highest correlation coefficient inTLC,FVC,FEV1,RV,FEV1/FVC were0.87,0.68,0.75,0.81,-0.64.4:The volume percentage of Rs,Rm,Ri,Ls,Li,RR and LL was20.51%,10.27%,23.10%,24.64%,21.49%,53.87%,46.13%.5:For few cases,the Li lobes inspiration lung volume were higher than the Lsabout25%(400ml) with no significant tumor affected ventilation or only withGGO condition,the LL inspiration volume were higher than the RR about16%(339ml).6: Human lung density distribution was not uniform on supine location,theminimum lung density for superior lobe was-870±21.9HU,the maximum lungdensity for inferior lobe was-767±16.3HU,P=0.0008(<0.01), there wasstatistically significant difference between groups.Conclusions】Utilized the preoperative data acquired by dual source CT quantificationvolume imaging to predict lung cancer patients’ postoperative pulmonary functionalteration was feasible,it could provide precise predictive value of these patientswho accepted different lobectomy, At the same time it could provide individualassessment so as to reduce the operation risk in special patients with lungphysiology index heteromorphosis,dual source CT had Obvious advantageincluding the faster scanning speed,the shorter imaging time,the better imagequality, so this technology had important clinical application value.
