Objective:To observe the pathological process in patients of COPD related pulmonary hypertension,the role of resistin-like molecule β(RELMβ) regulates PI3K/Akt/m TOR signaling pathway expression in pulmonary vascular and to investigate the role of those proteins in pulmonary vascular remodeling. Through tests the plasma of RELMβ and HIF-1a proteins in patients of COPD related pulmonary hypertension,to investigate the relationships between those proteins and pulmonary arterial systolic pressure(PASP),then provide a new theoretical support for COPD related pulmonary hypertension. Methods:There are two parts of this experiment.The first part of this experiment is to collect thirty lung tissue samples of the patients who was diagnosed with squamous cell carcinoma of lung to do Lobectomy.Through the tests of arterial blood gas analysis,pulmonary function testing and ultrasonic cardiogram,they are divided three groups,the first group is control group(Control),the second group is COPD group(COPD),the third group is COPD related pulmonary group(PH).To statistics the data of pulmonary vascular remodeling,through the Immunohistochemistry(IHC) to detect the protein of RELMβ expression,western blot to detect the proteins of RELMβ and the PI3K/Akt/m TOR signaling pathway expression in pulmonary vascular.The study is to analysis the roles of those proteins in the pathological process of pulmonary vascular remodeling.The second part of this experiment is to collect eighty blood sam ples of the patients who was diagnosed with acute exacerbation of chronic obstructive pulmonary disease.Through the tests of arterial b lood gas analysis,pulmonary function testing and ultrasonic cardiogra m,they are divided two groups,the first group is COPD group(COP D),the second group is COPD related pulmonary group(PH). The co ntrol group are forty volunteers from physical examination departme-nt, all of them reception the tests of arterial blood gas analysis,pul monary function testing and ultrasonic cardiogram.Collection about 5ml venous blood from everyone,plasma was separated by centrifuge,to detect the proteins of RELMβ and HIF-1a exrpession in plasma by ELISA and investigate the relationships between those proteins with PASP. Results: 1.Microscopic observation of the pulmonary vascular remodeling;(1)Under the microscopic observation of the control group, we co uld see the structural of alveolar clearly and thin pulmonary vascu lar wall thickness, there was no stenosis of the vessel lumen.(2)Under the microscopic observation of the COPD group,we coul d see the wall of alveolar become more thinner than the control gr oup,the structural of alveolar was damaged obviously and bullae of the lung form,there was stenosis of the vessel lumen. The percentag e of pulmonary vascular wall thickness and the percentage of pulm onary vascular wall area in COPD group were statistically significa ntly compared with the control group(P<0.05).But the percentage of pulmonary vascular lumen area in COPD group was significantly de creased compared with the control group(P<0.05).(3)Under the microscopic observation of the PH group,there was stenosis of the vessel lumen obviously and bullae of the lung form.The percentage of pulmonary vascular wall thickness, the percentage of pulmonary vascular wall area and the percentage of pulmonary v ascular lumen area in PH group were statistically significantly comp ared with the COPD group(P<0.05). 2.Immunohistochemical detect the protein of RELMβ expression in pulmonary vascular wallThe result of Immunohistochemical showed:The protein of RELMβ highly expression in COPD group,it was statistically significantly compared with the control group(P<0.05);the protein of RELMβ hig hly expression in PH group,it was statistically significantly compare d with the COPD group(P<0.05). 3.The proteins of RELMβ and PI3K/Akt/m TOR signaling pathway e xpression in patients of every group were detected by western blotThe results of western blot showed:The protein of RELMβ highly expression in COPD group,it was statistically significantly compare d with the control group(P<0.05);the protein of RELMβ highly expr ession in PH group,it was statistically significantly compared with t he COPD group(P<0.05);the protein of p-PI3 K highly expression in COPD group,it was statistically significantly compared with the cont rol group(P<0.05); the protein of p-PI3 K highly expression in PHgr oup,it was statistically significantly compared with the COPD group(P<0.05);the protein of p-Akt308 highly expression in COPD group,it was statistically significantly compared with the control group(P<0.05); the protein of p-Akt308 highly expression in PH group,it w as statistically significantly compared with the COPD group(P<0.05);the protein of p-Akt473 highly expression in COPD group,it wasstat istically significantly compared with the control group(P<0.05); the protein of p-Akt473 highly expression in PH group,it was statisticall y significantly compared with the COPD group(P<0.05);the protein of p-m TOR highly expression in COPD group,it was statistically significantly compared with the control group(P<0.05); the proteinof pm TOR highly expression in PH group,it was statistically significantl y compared with the COPD group(P<0.05).Pearson correlation analysis showed:The protein of RELMβ in pat ients pulmonary was positively correlated with WT%,WA% and PA SP,it was negatively correlated with LA% and Pa O2。 4.Detection the proteins of RELMβ and HIF-1a expression in plasm a of patientsThe expression of RELMβ in patients plasma of COPD group w as statistically significantly compared with the control group(P<0.05);the expression of RELMβ in patients plasma of PH group was stati stically significantly compared with the COPD group(P<0.05); the e xpression of HIF-1a in patients plasma of COPD group was statisti cally significantly compared with the control group(P<0.05);the expr ession of HIF-1a in patients plasma of PH group was statistically si gnificantly compared with the COPD group(P<0.05).Pearson correlation analysis showed:The protein of RELMβ in pat ients plasma was positively correlated with PASP and HIF-1a,it was negatively with Pa O2;the protein of HIF-1a in patients plasma was positively correlated with PASP,it was negatively correlated with P a O2.1. The protein expression of RELMβ was significant in patients pulmonary vascular wall of COPD related pulmonary hypertension,and it could mediating PI3K/Akt/m TOR signaling pathway to promote pul monary vascular remodeling. Which lead to pulmonary hypertension ev entually.2.
