| Objective:To study the meaning of combination Ubiquitin-52 ribosome fusion proteins with urine β2 Microglobulin to diagnosis early diabetic nephropathy (DN).Methods:from 2013 to 2014, choosing a random sample of Some patients with type 2 diabetes in our hospital of nephrology and endocrinology.The plasma glucose level of objects controlled good for nearly 3 month. ACR as the gold standard in the diagnosis of DN, patients with ACR in 30～300 mg/g diagnosis of DN, patients with ACR less than 30 mg/g diagnosis of DM.All patients was detected in urineUbA52 and P2 Microglobulin, fasting glucose, glycosylated hemoglobin. Calculated sensitivity, specificity and related indicators of urine UbA52 and (or) beta 2 microglobulin in patients with early DN.Results:In the joint diagnostic tests, diagnosis efficiency of Urine UBA52 and β2M’series test is best, Sensitivity of 84%(126/150),Specific degree of 94%(188/200),The missed diagnosis of 16% (24/150),Misdiagnosis rate was 6%(12/200),YOUDEN index is 0.78, Coarse coincidence rate 89.7% (314/350),14 (0.84/0.06) positive likelihood ratio and negative likelihood ratio of 0.17 (0.16/0.94), kappa value was 0.788, the area under the ROC curve is 0.89, positive predictive value of 0.913, negative predictive value of 0.887.Conclusion:1. Urine UbA52, β2 Microglobulin as an index of early diagnosis of diabetic nephropathy, its degree of sensitivity, specific, positive likelihood ratio and other indexes is better.2. Some residents to see a doctor in the hospital the urban area of diabetes history of 5 years of screening and diagnosis of diabetic nephropathy, UbA52 can be used as diagnostic indicators, the critical value of 6.2 mg/L;β2M can be used as diagnostic indexes, the optimal threshold is 0.96 mg/L.3. Area under the ROC curve of Series diagnostic test of Urine UbA52 and β2M’ is the largest, diagnosis efficiency is highest, the diagnosis of early diabetic nephropathy is that when UbA52 is greater than 6.2 mg/L and beta 2 m more than 0.96 mg/L. |
