Objective:To study the value of a new measurement that divided obstructive sleep apnea hypopnea syndrome(OSAHS) into rapid-eye-movement(REM) related and non-rapid-eye-movement(NREM) related subgroups. Methods:polysomnography (PSG) recordings of 45 males with severe OSAHS, Calculate the apnea hypopnea index(AHI) during the REM sleep(AHI1) and NREM sleep(AHI2), according to the ApneaGraph, calculate the apnea hypopnea index during the upper(AHI3) and lower(AHI4) of transpalatal level, REM predominance index was determined using AHI1/AHI2, of one’s total amount of obstructive events, the proportion of transpalatal level or tongue level, which was determined using AHI3/AHI3+ AHI4.A correlation analysis was performed between the sleep stage and the percentage of transpalatal level obstructions measured by AG. Results:1. There were 13 cases defined as REM-related OSAHS(28.89%)[REM apnea hyponea index (AHI)/NREM AHI>2] and 32 cases defined as NREM-related OSAHS(71.11%) [REM apnea hyponea index (AHI)/NREM AHI<2]; 2. Of the total amount of apnea hypopnea,the portions of AHI、the lowest arterial oxygen saturation and the different proportion (>60%and<60%) of AHI during the upper and lower of transpalatal level have no discrepancy (t=0.407,0.033, 0.502,0.532, P>0.05); 3. Correlation between the AHI were measured by the PSG and AG was significant (r=0.545, P=0.000<0.05); 4.When the proportion of transpalatal level>70%, correlation between the REM predominance index and localization of upper airway obstructive site was no significant(Z=2.669 P=0.102>0.05); When the proportion of transpalatal level>60%, correlation between the REM predominance index and localization of upper airway obstructive site was significant(χ2=5.543 P=0.019< 0.05); 5. The different Friedman type(FriedmanI+II and FriedmanⅢ+Ⅳ) between the REM and NREM was significant(χ2=4.409 P=0.036<0.05). Conclusion:For the males severe OSAHS, in particular the REM-related who had a correlation with upper airway pressure measurements indetermination of the main localization of upper airway obstructive site identified by upper airway pressure measurements during the sleep.
