Backgroud:Diabetic nephropathy is one of diabetic microvascular complications andmainly refers to glomerular sclerosis caused by diabetes. It has become increasingly evident that early phenomenon of type 2 diabetic nephropathy is kidney volume increase, glomerular filtration rate increase, and then intermittent proteinuria and microalbuminuria, finally renal insufficiency. It is one of the major cause of death in diabetes. The ultrastructure change of glomerular cell (podocyte) and the change in related molecular expression play an important role in the intial stage and development of diabetic nephropathy proteinuria. The proposed research intends to evaluate the effects compound rhodiola on the ultrastructure change of podocyte with type 2 diabetic nephropathy by animal and transmission electron microscope. In order to explore the mechanism of the kidney protection and offer new ideas and new methods for clinical treatment of DN.Objectives:This study aims to investigate the effect of compound rhodiola on blood glucose, blood lipid, renal function, advanced oxidation protein products (AOPPs), microalbuminuria, body weight, the ultrastructure change of podocyte with type 2 diabetic nephropathy, in order to find out effectively methods to control the DN patients.Methods:60 healthy eight-week-old SPF-Wistar were randomly divided into five groups:the normal control group (n=10) and the high sugar and fat model group (n=40). Then the type 2 diabetic nephropathy rats were divided into four groups:type 2 diabetic nephropathy controlled group, rhodiola groups (low, medium and high dose). After 4weeks, the fasting blood were collected, and levels of FBG, mALB, AOPPs were determined. Under anesthesia, the renal cortex under 2-10 mm were collected and fixed in cryogenic liquid, immediately sended to Xinxiang medical electron microscope room to determine pathological podocyte count, foot average width and foot fusion rate, average thickness of basement membrane.Results:1. Compared with the blank group, the weight of type 2 diabetic nephropathy controlled group significantly decreased (P<0.01). The weight of medium dose rhodiola group rats was decreased (P<0.05). Compared with the type 2 diabetic nephropathy controlled group, the weight of low dose compound rhodiola group rats were very significantly higher (P<0.01), high dose rhodiola group rats were higher(P<0.05). Among the three dose groups, the low dose rats had the highest weight.2. Compared with blank group, the mALB of low and medium dose rhodiola groups had a significantly difference (P<0.05), while high dose had a more degree difference(P<0.01). Compared with the type 2 diabetic nephropathy controlled group, all the rhodiola groups had a very significantly difference (P<0.01). The three rhodiola groups had no difference.3. Compared with the type 2 diabetic nephropathy controlled group, the AOPPs of all the rhodiola groups and blank controlled group had a very significantly difference(P<0.01)). The AOPPs of all the rhodiola groups and blank controlled group had no difference.4. The pathological podocyte count of the type 2 diabetic nephropathy controlled group significantly lower than other groups and the foot average width, foot fusion rate, average thickness of basement membrane were increased. All the indicators were the same between medium and high dose rhodiola group. The pathological podocyte count increased and the foot average width, foot fusion rate, average thickness of basement membrane decreased (P<0.05) in the rhodiola groups.Conclusion:1. Rhodiola can effectively decrease type 2 diabetic nephropathy rats fasting blood glucose and proteinuria.2. Rhodiola can improve the ultrastructure change of podocyte and the pathological podocyte count decrease the foot average width, foot fusion rate, average thickness of basement membrane with type 2 diabetic nephropathy. Rhodiola can protect kidney.
