The Value Of Computer Tomography In Diagnosing TNM Staging Of Non-small Cell Lung Cancer

Objective:Comparison of non-small cell lung cancer patients with preoperative CT clinical TNM staging and the postoperative pathological diagnosis of TNM sta ging results,Evaluation of clinical and pathological TNM cancer staging consistency and to explore its causes.Methods:A retrospective study of Xinjiang Tumor Hospit al from January 2014 to August 2014 underwent surgical treatment of 92 patients with non-small cell lung cancer,and all patients underwent chest CT scan before op eration,according to the latest UICC lung cancer TNM staging standards (2009 editi on) for clinical and pathologic TNM staging respectively,consistency with Kappa st atistic test of two stage result,and to evaluate the sensitivity, specificity and accurac y of preoperative CT diagnosis of lymph node metastasis in each group.Results:Cli nical T stage and postoperative pathologic T stage of the satisfactory consistency (Kappa value=0.727,P<0.05), the coincidence rate was 83.7%;Consistency checking satisfaction with pathologic N stage and clinical N stage was acceptable (kappa v alue=0.635,P<0.05),the coincidence rate was 79.3%;Clinical TNM staging and surgi cal pathological TNM staging consistency test results show the basic satisfaction (Kappa value=0.680,P<0.05),the coincidence rate was 75.0%.The sensitivity,specifici ty,positive predictive value,negative predictive value,accuracy rate of CT in diagnosi s of lymph node metastasis were71.1%,85.2%,77.1%,80.7%,79.3%.Preoperative CT d iagnosis of 4,5,6,10 group with a relatively low accuracy of lymph node metastasis, in which four groups of lymph node metastasis of high false positive,false negative 10 lymph node metastases is higher.Conclusion:CT diagnosis of clinical TNM sta ging and pathologic TNM staging consistency was satisfactory,the preoperative CT examination for non small cell lung cancer TNM staging provides important clinica 1 reference value,but there are certain limitations,if necessary,combined with other te sts such as PET/CT to further improve the accuracy of clinical TNM staging.
