| Objective:Splenectomy Cardia peripheral vascular amputation and treatment of partial splenic embolization of liver cirrhosis and portal hypertension recent clinical efficacy.Methods:January 2012-December 2014 period, the First Affiliated Hospital of Xinjiang Medical University because of cirrhosis and portal hypertension treatment of 84 patients, liver function Child-Pugh grade:A grade of 16 (19.04%); Class B 45 people (53.57%), C grade 23 people (27.39%). Partial splenic artery embolization. Postoperative patients three days, seven days a WBC, RBC, PLT, ALT, AST and TBIL count; portal pressure after a week; postoperative complications; postoperative hospital stay and hospital costs.Results:Splenectomy Cardia peripheral vascular amputation group after three days off peripheral blood count:WBC:(14.56 ± 6.60)×109/L, RBC:(3.64 ±0.81) ×1012/L, PLT:(148.90 ± 67.74)×109/L; partial splenic artery embolization group after three days peripheral blood count:WBC:(7.55±4.29)×109/L, RBC:(3.28±0.88)× 1012/L, PLT:(95.33 ± 47.71)×109/L. Splenectomy Cardia peripheral vascular amputation group after seven days off peripheral blood count:WBC:(8.85 ± 3.29) ×109/ L, RBC:(3.94±1.13)×1012/L, PLT:(329.68±157.63)×109/L; partial splenic artery embolization group after 7 days peripheral blood count:WBC:(6.25±2.49)×109/L, RBC:(3.49±1.01)×1012/L, PLT:(161.19±58.13) ×109/L. Splenectomy Cardia peripheral vascular amputation group after three days ALT U/L 53.59 ±40.32, AST U/L 71.21±32.93.17; TBIL (μmol/L) 35.03 ± 19.25; after seven days ALT U/L 40.22 ± 18.17, AST U/L 42.18 ± 17.20; TBIL (μmol/L) 27.14±12.44; partial splenic artery embolization group:after three days ALT U/L 47.57 ± 22.05, AST U/L 63.81 ± 27.97; TBIL30.12 ± 11.43; after seven days ALT U/L 37.20 ± 8.85, AST U/L 44.29 ± 12.01; TBIL (μmol/L) 16.36 ± 4.07; Splenectomy Cardia peripheral vascular amputation group:nausea, vomiting, fever, abdominal pain, bloating and other symptoms (52 cases); pleural effusion (15 cases); lower left pneumonia (7 cases); ascites patients (14 cases); portal vein thrombosis formation (two cases); postoperative abdominal hemorrhage (n= 1); postoperative death (one case). Partial splenic artery embolization group:Almost all patients after varying degrees of nausea, vomiting, fever, postoperative patients with pleural effusion (9 cases); lower left pneumonia (three cases); ascites patients (11 cases); portal vein thrombosis formation (one case); spleen liquefaction necrosis (1 case). Splenectomy Cardia peripheral vascular amputation group, the average hospital stay was:14.5 ± 4.51 days, the average hospital cost:46955.40 ± 15122.50 yuan; partial splenic artery embolization average number of days of hospitalization:13 ± 4.22 days, the average hospital cost:38429 ± 10122.02 yuan.Conclusion:Splenectomy Cardia peripheral vascular amputation group is better than the recent partial splenic artery embolization group. |
