Objective To study the clinical features of patients with somatoform disorders, and explore the therapeutic effects of duloxetine on somatoform disorders.Methods(1)70 patients with somatoform disorders, who came to our hospital from April 2013 to May 2014 and met our criteria, were chosed as study group. And 70 healthy adults were chosed as control group. All subjects accepted some scales assessment, which include self-made general data questionnaire, MMPI, TCSQ, LES, TAS, SOMS-7.(2)The study group were treated with duloxetine for 8 weeks. Before treatment and in the ends of 4, 8 weeks the patients were evaluated by HAMA, HAMD and SOMS-7, and TESS in the ends of 2, 4, 6,8 weeks. The score reduction ratio of HAMA was thought as the main method of evaluation, which was acquired by(the score before treatment- the score after treatment)/the score before treatment × 100%. It was divided to four grades: healing(the reduction ratio≥75%), significantly progressive(50%-74%), progressive(25%-49%) and invalid(the ratio < 25%). All the data were managed by SPSS 16.0 software. We used T-test, Chi-squate test and anova for univariate analysis while Pearson test for correlation analysis. When P<0.05 it was confirmed significant.Results1.General data:Finally 70 patients in study group and 70 in control group finished the whole process. The number of being married in study group was less than that in controls significantly while the number of being unmarried, divorced, widowed and living alone was more.2.Clinical features: There were significant differences in F, Hs, D, Hy, Pd, Pa, Sc and Si sections in the MMPI score comparison between two groups. Furthermore the score of the sections in study group was higher than that in control groups. TCSQ analysis demonstrated that the negative score of study group was higher than control group significantly and the positive score was lower. LES analysis demonstrated that there were significant differences between two groups in all parts of LES. Except positive events stimulation the other parts scores in study group were higher than control group. TAS score analysis indicated that the scores of factor 1, 2, 4 and total score in study group were higher significantly than control group. SOMS-7 analysis indicated that the symptom number of study group was 15.06±4.91 before treatment, and severity was 28.04±13.53. Malaise was the most frequent symptom. Before treatment HAMA score of study group was 25.27±4.32.3.Prognosis results: After the treatment of duloxetine for 4 weeks, it improved significantly in somatozation symptoms number and severity, HAMA score and HAMD score. What’s more, it is also significantly on the end of 8 weeks. The reduction ratio results of HAMA total score suggested that 3 patients rescure(4.3%), 21 significant improvement(30.0%), 26 improvement(37.1%), and 20 invalidity(28.6%).TESS analysis suggested that 24 patient had side effects. The ratio of patient with side effects was 34.3%.Conclusions1.The majority patients with somatoform disorder were female, family breakdown and old. 2.There were abnormal personality, alexithymia, anxiety, depression, hypochondriasis. The patients were affected more by negative events and apt to give negative coping. 3.Duloxetine have good effects on somatoform disorders. In addition, duloxetine had modicous side effects.
